Description
Overview of the medicine
Aceta-Extend 665 mg Tablet is an extended-release formulation of acetaminophen, used for the relief of mild to moderate pain and to reduce fever. Its extended-release characteristic provides longer-lasting pain relief.
Uses & Indications
Dosage
Adults
Typically, 2 tablets (1330 mg) every 8 hours, not to exceed 6 tablets (3990 mg) in 24 hours.
Elderly
Similar to adult dosage, but caution with hepatic or renal impairment. Consult a physician.
Renal_impairment
Dosage adjustment may be necessary in severe renal impairment. Consult a physician.
How to Take
Take orally with a full glass of water. Do not crush, chew, or dissolve the tablet, as this may destroy the extended-release mechanism.
Mechanism of Action
Acetaminophen primarily acts in the central nervous system by inhibiting prostaglandin synthesis, which is responsible for its analgesic and antipyretic effects. It also blocks pain impulse generation peripherally to a lesser extent.
Pharmacokinetics
Onset
Onset of analgesic action typically within 30-60 minutes, with sustained action due to extended release.
Excretion
Excreted primarily in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma half-life of 2-4 hours, but the extended-release formulation extends the therapeutic effect.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved slower with extended-release formulations, typically within 2-4 hours, providing sustained release.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by the cytochrome P450 enzyme system to a toxic intermediate (NAPQI), which is normally detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Hypersensitivity to acetaminophen or any component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Concurrent use with alcohol increases the risk of hepatotoxicity.
Other hepatotoxic drugs
May increase the risk of liver damage when co-administered.
Warfarin and other anticoagulants
May increase the anticoagulant effect, leading to an increased risk of bleeding, especially with prolonged daily use.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, abdominal pain, and sweating, which may progress to liver failure. Severe overdose can be fatal. Management involves immediate medical attention, gastric decontamination if recent ingestion, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe for use during pregnancy and lactation at recommended doses, but always consult a healthcare professional before use. Prolonged high-dose use should be avoided.
