Overview of the medicine
Acifix 20 mg tablet contains Esomeprazole, which is a proton pump inhibitor (PPI). It works by reducing the amount of acid your stomach makes, providing relief from symptoms such as heartburn, acid reflux, and helping to heal ulcers.
Uses & Indications
Dosage
Adults
GERD (Erosive Esophagitis): 20 mg or 40 mg once daily for 4 to 8 weeks. GERD (Symptomatic): 20 mg once daily for 4 weeks. H. pylori eradication: 20 mg twice daily for 7-14 days (in combination with antibiotics).
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential for reduced hepatic function.
Renal_impairment
No dose adjustment is required in patients with renal impairment.
How to Take
Acifix tablets should be swallowed whole with water. Do not chew or crush. It is best taken at least one hour before a meal.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This irreversible binding blocks the final step of acid secretion, leading to a sustained reduction in gastric acid production.

Excretion
Approximately 80% of metabolites are excreted in the urine and 20% in the feces.
Half life
Approximately 1.5 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 1-2 hours. Bioavailability is approximately 64% after a single 20 mg dose, increasing to 89% with repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Esomeprazole, other substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR/PT has been reported, requiring monitoring.
Clopidogrel
Esomeprazole may reduce the antiplatelet activity of clopidogrel, especially in CYP2C19 poor metabolizers.
Methotrexate
May increase methotrexate levels, especially in high-dose therapy.
Atazanavir/Nelfinavir
Reduced plasma concentrations of atazanavir and nelfinavir due to increased gastric pH.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether esomeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
