Overview of the medicine
Aciphin 2 gm Injection contains Ceftriaxone, a third-generation cephalosporin antibiotic used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
Typical dosage is 1-2 gm once daily (or in two divided doses) via IV or IM route, depending on the severity of infection. Max 4 gm/day.
Elderly
No specific dose adjustment required unless severe renal or hepatic impairment.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment (CrCl < 10 mL/min) or dialysis, close monitoring and dose reduction may be considered.
How to Take
For IV administration, reconstitute with sterile water for injection and administer over 30 minutes. For IM, reconstitute with lidocaine 1% (without epinephrine) and administer deeply into a large muscle.
Mechanism of Action
Ceftriaxone inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.

Onset
Rapid onset of action, typically within 1 hour.
Excretion
About 50-60% excreted unchanged in urine via glomerular filtration, and 35-45% excreted via bile and feces.
Half life
Approximately 6-9 hours (longer in neonates and elderly).
Absorption
Well absorbed after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations reached within 2-3 hours after IM.
Metabolism
Not extensively metabolized; primarily converted to inactive metabolites in the gut flora.
Side Effects
Contraindications
- Hypersensitivity to ceftriaxone, any other cephalosporin, or penicillin
- Neonates with hyperbilirubinemia
- Premature neonates
- Concomitant use with IV calcium-containing solutions in neonates

Aminoglycosides
Synergistic effect, but potential for increased nephrotoxicity.
Calcium-containing solutions
Formation of ceftriaxone-calcium precipitates, especially in neonates; avoid concomitant administration.
Loop diuretics (e.g., Furosemide)

Storage
Store below 30°C in a dry place, away from light and moisture. Reconstituted solution is stable for 6 hours at room temperature or 24 hours under refrigeration.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. Hemodialysis and peritoneal dialysis are not effective for removing ceftriaxone.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Excreted in low concentrations in breast milk; use with caution in nursing mothers.
