Overview of the medicine
Aclene Plus Gel is a combination topical medication containing Adapalene (a retinoid-like compound) and Benzoyl Peroxide. It is primarily used for the treatment of acne vulgaris in patients 9 years of age and older. It works by reducing the formation of comedones and killing acne-causing bacteria.

Dosage
Adults
Apply a thin layer of the gel to affected areas of the face and/or trunk once daily in the evening after washing gently with a non-medicated cleanser and patting dry.
Elderly
No specific dosage adjustments are required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
For topical dermatological use only. Apply to clean, dry skin. Avoid contact with eyes, lips, nostrils, and mucous membranes. Wash hands thoroughly after application.
Mechanism of Action
Adapalene modulates cellular differentiation, keratinization, and inflammatory processes. It normalizes differentiation of follicular epithelial cells, resulting in decreased microcomedone formation. Benzoyl Peroxide is an oxidizing agent with bactericidal and keratolytic effects. It kills Propionibacterium acnes (now Cutibacterium acnes) by releasing free oxygen radicals and aids in shedding dead skin cells.
Pharmacokinetics
Onset
Significant improvement is typically observed after 1-4 weeks of daily application, with continued improvement over time.
Excretion
Adapalene is primarily excreted by the biliary route. Benzoic acid (from benzoyl peroxide) is excreted in the urine.
Half life
Due to minimal systemic absorption, plasma half-life values are not clinically relevant for topical administration.
Absorption
Systemic absorption of adapalene and benzoyl peroxide is minimal following topical application. Adapalene plasma concentrations are generally below the quantification limit (0.1 ng/mL). Benzoyl peroxide is rapidly metabolized to benzoic acid after skin penetration and then excreted.
Metabolism
Topically applied adapalene is metabolized to more polar conjugates, primarily via O-demethylation, in the liver. Benzoyl peroxide is converted to benzoic acid in the skin.
Side Effects
Contraindications
- Hypersensitivity to adapalene, benzoyl peroxide, or any component of the gel.

Drug Interactions
Topical preparations with irritating or drying effects
Avoid concomitant use as it may lead to cumulative irritation, especially with sulfur, resorcinol, or salicylic acid. If used, apply at different times of the day.
Products containing alpha hydroxy acids or glycolic acid
Concurrent use may increase skin irritation.
Storage
Store below 30°C in a dry place. Protect from light and freezing. Keep out of reach of children.
Overdose
Topical overdose may result in marked redness, peeling, or discomfort. Systemic effects from an accidental topical overdose are unlikely due to the minimal percutaneous absorption. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether adapalene or benzoyl peroxide is excreted in human milk after topical application; exercise caution when administering to a nursing mother. Consult a physician before use.
