Overview of the medicine
Exovate-N Ointment is a combination medication containing a potent corticosteroid (Clobetasol Propionate) and an aminoglycoside antibiotic (Neomycin Sulfate). It is used to treat inflammatory skin conditions where bacterial infection is present or suspected.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than 7 consecutive days unless advised by a doctor.
Elderly
Same as adults, but use with caution due to thinner skin and increased potential for systemic absorption.
Renal_impairment
No specific dosage adjustment for topical use; however, systemic absorption of neomycin could be a concern with severe impairment and large application areas.
How to Take
For external use only. Apply a small amount thinly to the affected skin area and gently rub in. Do not use on the face for prolonged periods. Avoid contact with eyes, mouth, and other mucous membranes. Wash hands before and after application.
Mechanism of Action
Clobetasol Propionate is a highly potent corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties. It works by inhibiting the release of mediators involved in inflammatory and allergic responses. Neomycin Sulfate is an aminoglycoside antibiotic that interferes with bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to bacterial death.

Onset
Topical effects usually begin within a few hours of application.
Excretion
Clobetasol metabolites are primarily excreted by the kidneys. Systemically absorbed Neomycin is excreted largely unchanged by the kidneys.
Half life
Systemically absorbed Clobetasol has a plasma half-life of approximately 4-5 hours. Systemically absorbed Neomycin has a half-life of 2-3 hours.
Absorption
Clobetasol Propionate can be absorbed systemically, especially with prolonged use, application over large areas, or under occlusive dressings. Neomycin Sulfate absorption through intact skin is minimal but increases significantly with damaged or broken skin.
Metabolism
Clobetasol is primarily metabolized in the liver to inactive metabolites. Neomycin is poorly metabolized.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate, Neomycin Sulfate, other corticosteroids, or any component of the formulation.
- Untreated bacterial, fungal, or viral infections (unless the infection is the specific target of the combination).
- Rosacea, acne vulgaris, perioral dermatitis, perianal and genital pruritus.
- Pruritus without inflammation.
- Dermatoses in infants under 1 year of age.
Drug Interactions
CYP3A4 Inhibitors
Systemic absorption of clobetasol may be increased by concomitant use of CYP3A4 inhibitors (e.g., ritonavir, itraconazole), leading to increased systemic corticosteroid effects.
Other Aminoglycosides
If significant systemic absorption of neomycin occurs (rare with topical use), there could be additive ototoxic or nephrotoxic effects with other systemically administered aminoglycosides.

Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely. Chronic overdose or misuse can lead to systemic corticosteroid effects (e.g., Cushing's syndrome, adrenal suppression) or local adverse effects such as skin thinning. If significant systemic absorption of neomycin occurs, ototoxicity or nephrotoxicity could be concerns.
