Description
Overview of the medicine
Aldonist 25 mg Tablet contains Spironolactone, a potassium-sparing diuretic. It is used to treat conditions such as heart failure, high blood pressure, fluid retention (edema) caused by various conditions like liver cirrhosis or nephrotic syndrome, and primary hyperaldosteronism. It helps the body get rid of excess water and sodium while retaining potassium.
Uses & Indications
Dosage
Adults
For heart failure, typically 25 mg orally once daily, may be increased to 50 mg daily. For hypertension, 25-100 mg daily. For edema, 25-200 mg daily in divided doses.
Elderly
Initiate with lower doses and titrate carefully, monitoring renal function and electrolytes.
Renal_impairment
Contraindicated in severe renal impairment. Use with caution in moderate impairment, requiring close monitoring of serum potassium and renal function.
How to Take
Oral administration. It is generally recommended to take Aldonist with food to enhance absorption and reduce gastrointestinal upset.
Mechanism of Action
Spironolactone acts as a competitive antagonist of aldosterone at the mineralocorticoid receptors in the distal renal tubules and collecting ducts. This action prevents sodium reabsorption and potassium excretion, leading to increased diuresis and decreased potassium loss.
Pharmacokinetics
Onset
Diuretic effect usually evident within 2-3 days; antihypertensive effect may take 2-4 weeks.
Excretion
Primarily excreted in urine and feces as metabolites.
Half life
Spironolactone (parent drug): 1.3 hours; Active metabolites (Canrenone): 10-35 hours.
Absorption
Rapidly but incompletely absorbed from the gastrointestinal tract; absorption is increased when taken with food.
Metabolism
Extensively metabolized in the liver to active metabolites, primarily canrenone.
Side Effects
Contraindications
- Hyperkalemia
- Severe renal impairment or acute renal failure
- Anuria
- Addison's disease
- Concomitant use with eplerenone or other potassium-sparing diuretics.
Drug Interactions
Digoxin
Spironolactone may increase serum digoxin concentrations, leading to toxicity.
Lithium
Increased lithium levels due to reduced renal clearance.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce diuretic and antihypertensive effects; increased risk of renal dysfunction.
ACE inhibitors, Angiotensin Receptor Blockers (ARBs)
Increased risk of hyperkalemia.
Potassium supplements, other potassium-sparing diuretics
Severe hyperkalemia.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, confusion, nausea, vomiting, dizziness, and diarrhea. Hyperkalemia is a major concern, potentially leading to cardiac arrhythmias. Treatment involves discontinuing the drug, gastric lavage, supportive measures, and management of hyperkalemia.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Spironolactone metabolites are excreted in breast milk; use with caution in nursing mothers or consider an alternative.

WHO Essential Medicine
YesClinical Trials
Spironolactone has been extensively studied in clinical trials, particularly for its role in heart failure (e.g., RALES trial), demonstrating significant reductions in mortality and morbidity.
Lab Monitoring
- Regularly monitor serum potassium levels, renal function (creatinine, BUN), and electrolytes. Blood pressure monitoring is also crucial.
Doctor Notes
- Crucial to monitor serum potassium and renal function (creatinine) at initiation and during dose adjustments, especially in patients at high risk of hyperkalemia.
- Advise patients to avoid potassium supplements and potassium-rich foods.
- Counsel male patients regarding the possibility of gynecomastia.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor.
- Do not stop taking it without consulting your doctor.
- Avoid consuming potassium supplements or high-potassium foods.
- Report any signs of hyperkalemia such as muscle weakness, irregular heartbeat, or numbness.
- Inform your doctor about all other medications you are taking.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Aldonist may cause dizziness or drowsiness, especially when first starting the medication or adjusting the dose. Exercise caution when driving or operating machinery until you know how the medicine affects you.
Lifestyle Advice
- Maintain a healthy diet, avoiding excessive potassium intake.
- Limit alcohol consumption.
- Stay hydrated, but follow your doctor's advice on fluid intake if you have heart failure.
- Engage in regular light exercise if appropriate.
