Alkriz 250 mg Capsule is a macrolide antibiotic used to treat a wide range of bacterial infections, including those of the respiratory tract, skin, and soft tissues. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
250-500 mg twice daily for 7-14 days, depending on the type and severity of infection. For H. pylori eradication, typically 500 mg twice daily for 7-14 days as part of a triple therapy regimen.
Elderly
No specific dosage adjustment is usually required unless severe renal impairment is present.
Renal_impairment
For patients with severe renal impairment (creatinine clearance <30 mL/min), the dosage should be reduced by half or the dosing interval prolonged.
How to Take
Alkriz capsules should be taken orally, with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Alkriz inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translocation of peptides.

Onset of antibacterial action is typically within a few hours of administration.
Excretion
Excreted through both renal (approximately 20-40% unchanged drug and metabolite) and biliary/fecal routes (approximately 50-60%).

Well absorbed orally, approximately 50% bioavailability. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4, to an active 14-hydroxy metabolite which also has antibacterial activity.
Side Effects
Contraindications
- Hypersensitivity to macrolide antibiotics or any component of the formulation.
- Concomitant administration with cisapride, pimozide, astemizole, terfenadine, or ergot alkaloids (e.g., ergotamine, dihydroergotamine).
Drug Interactions
Digoxin
Increased serum digoxin concentrations.
Warfarin
Increased anticoagulant effect, monitor INR.
Theophylline
Increased serum theophylline concentrations.
Statins (e.g., simvastatin, lovastatin)
Increased risk of myopathy and rhabdomyolysis.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal upset (nausea, vomiting, diarrhea) and abdominal pain. Treatment involves gastric lavage and symptomatic and supportive care. Hemodialysis and peritoneal dialysis are not effective.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, caution is advised during breastfeeding.
