Description
Overview of the medicine
Amdopril 5 mg tablet contains Amlodipine Besilate, a long-acting calcium channel blocker primarily used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood, and increasing blood supply to the heart.
Uses & Indications
Dosage
Adults
Initial dose is 5 mg once daily. May be increased to a maximum of 10 mg once daily depending on patient response and tolerability.
Elderly
Initial dose of 2.5 mg once daily is recommended.
Renal_impairment
No dosage adjustment is generally required.
How to Take
Take orally once daily, with or without food. It is recommended to take the tablet at about the same time each day.
Mechanism of Action
Amlodipine selectively inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. It acts predominantly on vascular smooth muscle, causing vasodilation and reducing peripheral vascular resistance. In angina, it reduces myocardial oxygen demand by dilating peripheral arterioles and arteries and increases myocardial oxygen delivery by dilating coronary arteries.

6-12 hours for antihypertensive effect.
Excretion
Primarily excreted as inactive metabolites in the urine (approximately 60%) and feces (approximately 25%).
Half life
30-50 hours (prolonged in hepatic impairment and elderly).
Absorption
Well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring 6-12 hours after oral administration. Bioavailability is 64-90%.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approximately 90%).
Side Effects
Contraindications
- Hypersensitivity to Amlodipine, dihydropyridine derivatives, or any component of the formulation.
- Severe hypotension.
- Cardiogenic shock.
- Obstructive outflow tract disease (e.g., severe aortic stenosis).
- Hemodynamically unstable heart failure after acute myocardial infarction.
Drug Interactions
Tacrolimus
Increased blood levels of tacrolimus when co-administered with amlodipine.
Simvastatin
Co-administration with Amlodipine may increase simvastatin exposure, increasing the risk of myopathy/rhabdomyolysis. Limit simvastatin dose to 20 mg daily.
Clarithromycin
Increased risk of hypotension and acute kidney injury with concomitant use.
CYP3A4 Inducers (e.g., Rifampicin)
May decrease Amlodipine exposure, potentially reducing its effectiveness.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase Amlodipine exposure, leading to increased risk of hypotension.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves close monitoring, symptomatic treatment, and supportive measures, including vasopressors and intravenous fluids. Calcium gluconate may be administered.
Pregnancy & Lactation
Pregnancy: Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Lactation: Amlodipine is excreted in human milk. Discontinue nursing or the drug, taking into account the importance of the drug to the mother.
