Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This antibiotic treats only bacterial infections. It will not work for viral infections (such as common cold, flu).
Uses & Indications
Dosage
Adults
Adults: 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the severity and type of infection. (Note: 125 mg suspension is typically for pediatric use).
Elderly
Similar to adult dosage; however, dose adjustments may be necessary based on renal function.
Renal_impairment
Dosage adjustment is required for patients with severe renal impairment (creatinine clearance <30 mL/min). Consultation with a doctor is essential.
How to Take
Take this medicine by mouth, usually every 8 or 12 hours, with or without food, as directed by your doctor. Shake the bottle well before each dose. Measure the liquid with a special measuring spoon or device. Do not use a household spoon. Complete the full course of treatment, even if symptoms improve.
Mechanism of Action
Amoxicillin is a bactericidal antibiotic belonging to the beta-lactam class. It inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to lysis and death of the bacterial cell.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour.
Excretion
Primarily excreted unchanged in the urine (50-70%) by tubular secretion and glomerular filtration within 6 hours. Small amounts are excreted in feces.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, especially when taken orally. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Approximately 10-25% is metabolized in the liver to inactive metabolites. The major metabolite is penicilloic acid.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin or any other penicillin antibiotic.
- History of severe hypersensitivity reaction to any other beta-lactam agent (e.g., cephalosporins, carbapenems, monobactams).
Drug Interactions
Warfarin
May increase the anticoagulant effect, leading to increased bleeding risk.
Allopurinol
Increased risk of skin rash when co-administered with amoxicillin.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce the effectiveness of oral contraceptives, advising additional contraceptive methods.
Storage
Store the dry powder at room temperature (below 30°C). Keep away from moisture and direct sunlight. After reconstitution, store the suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. In severe cases, renal failure and convulsions may occur. Treatment is generally supportive and symptomatic. Gastric lavage may be considered in recent ingestions. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Amoxicillin is generally considered safe for use during pregnancy (Category B). Small amounts are excreted in breast milk, but it is generally considered compatible with breastfeeding. Consult your doctor before use if you are pregnant or breastfeeding.
