Mechanism of Action

15-30 minutes
Excretion
Mainly excreted in the urine as metabolites.
Half life
Approximately 11-15 hours (range 6-27 hours)
Absorption
Rapidly and well absorbed after oral administration. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver primarily by cytochrome P450 3A4 (CYP3A4) to an active alpha-hydroxyalprazolam and an inactive benzophenone derivative.
Side Effects
Contraindications
- Hypersensitivity to alprazolam or other benzodiazepines
- Acute narrow-angle glaucoma
- Concomitant use with potent CYP3A inhibitors (e.g., ketoconazole, itraconazole)
Drug Interactions
Opioids
Increased risk of respiratory depression, profound sedation, coma, and death. Avoid concomitant use or limit dosage and duration.
CNS Depressants (e.g., Alcohol, Antidepressants, Antihistamines)
Additive CNS depressant effects, increasing drowsiness and sedation.
CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole, Grapefruit juice)
Can significantly increase alprazolam plasma concentrations, potentially leading to increased side effects and toxicity.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, impaired coordination, diminished reflexes, and coma. Treatment is primarily supportive. Flumazenil, a benzodiazepine receptor antagonist, may be used to reverse effects but can precipitate acute withdrawal in dependent patients.
Pregnancy & Lactation
Pregnancy Category D. Alprazolam can cause fetal harm when administered to a pregnant woman. Avoid during pregnancy. It is excreted in human milk and may cause sedation in the infant; discontinue nursing or the drug.
