Overview of the medicine
Apozyth 200 mg Suspension is a macrolide antibiotic used to treat various bacterial infections in children and adults.
Uses & Indications
Dosage
Adults
500 mg (12.5 ml of suspension) as a single dose on day 1, followed by 250 mg (6.25 ml) once daily for 4 days, or 500 mg (12.5 ml) once daily for 3 days.
Elderly
No dosage adjustment is generally required for elderly patients.
Pediatric
10 mg/kg once daily for 3 days, or 10 mg/kg on day 1 followed by 5 mg/kg once daily on days 2-5 (maximum 500 mg/day).
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Shake the bottle well before each use. Administer orally with the provided measuring device. Can be taken with or without food.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with mRNA translation.

Excretion
Mainly excreted unchanged in bile; a small portion is excreted via urine.

Approximately 2-4 days (due to extensive tissue distribution and slow release).
Absorption

Primarily hepatic, but not via the cytochrome P450 system to a significant extent.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased plasma digoxin levels. Monitor digoxin levels.
Warfarin
Increased anticoagulant effects. Monitor PT/INR.
Nelfinavir
Increased azithromycin concentrations. Monitor for azithromycin side effects.
Cyclosporine
Increased cyclosporine levels. Monitor cyclosporine levels.
Antacids (containing aluminum or magnesium)
Decreased peak plasma levels of azithromycin when co-administered. Take azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store the dry powder below 30°C. Protect from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C) and discard after 5-10 days.
Overdose
In case of overdose, symptoms are typically exaggerated side effects such as severe nausea, vomiting, and diarrhea. Gastric lavage and general supportive measures are recommended.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human milk; use with caution in nursing mothers.
