Description
Overview of the medicine
Arilol 25 mg Tablet is a cardioselective beta-adrenoceptor blocker used to treat various cardiovascular conditions, including hypertension, angina pectoris, and certain cardiac arrhythmias. It works by blocking beta-receptors in the heart, leading to a reduction in heart rate and blood pressure.
Uses & Indications
Dosage
Adults
Hypertension: Initially 25 mg once daily, may be increased to 50 mg if necessary after 1-2 weeks. Angina: 25-50 mg once daily. Arrhythmias: 25-50 mg once daily.
Elderly
Start with a lower dose, typically 12.5 mg once daily. Adjust dose carefully based on response and renal function.
Renal_impairment
Dose adjustment required. For CrCl 15-35 mL/min, 25 mg every other day. For CrCl <15 mL/min, 25 mg every four days.
How to Take
Take orally, with or without food, preferably at the same time each day. Do not crush or chew the tablet; swallow whole with water.
Mechanism of Action
Arilol selectively blocks beta-1 adrenergic receptors, primarily located in the heart. This reduces heart rate, myocardial contractility, and cardiac output, leading to a decrease in blood pressure. It also reduces renin release from the kidneys.
Pharmacokinetics
Onset
Within 1-2 hours
Excretion
Mainly via renal excretion (approximately 85-100% unchanged drug within 24 hours).
Half life
Approximately 6-9 hours
Absorption
Rapid and complete absorption from the gastrointestinal tract (approximately 80-90%). Bioavailability is about 50-60% due to first-pass metabolism.
Metabolism
Minimal hepatic metabolism (less than 10%), primarily excreted unchanged.
Side Effects
Contraindications
- Second or third-degree heart block (without a pacemaker)
- Cardiogenic shock
- Uncontrolled heart failure
- Severe bradycardia
- Sick sinus syndrome (without a pacemaker)
- Severe peripheral arterial disease
Drug Interactions
Digoxin
May increase the risk of bradycardia.
Clonidine
Withdrawal of clonidine may cause rebound hypertension, especially if a beta-blocker is also withdrawn abruptly.
NSAIDs (e.g., ibuprofen)
May reduce the hypotensive effect of beta-blockers.
Calcium Channel Blockers (e.g., verapamil, diltiazem)
Increased risk of bradycardia, hypotension, and heart block. Careful monitoring is required.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, acute heart failure, and bronchospasm. Management involves symptomatic and supportive treatment, including atropine for bradycardia, vasopressors for hypotension, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C. Arilol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when administered to a nursing mother.
