Description
Overview of the medicine
Atropine Sulfate Ophthalmic Solution is an anticholinergic agent. Low dose (0.01%) is specifically used for the control of myopia progression in children and adolescents.
Uses & Indications
Dosage
Adults
Not typically used for myopia control in adults. For other indications with higher concentrations, consult a physician.
Elderly
Dosage adjustment not typically specified for ophthalmic use; caution with systemic anticholinergic effects.
Renal_impairment
No specific dosage adjustment recommended for topical ophthalmic use.
Children_adolescents
One drop in each eye once daily at bedtime.
How to Take
For ophthalmic use only. Instill in the conjunctival sac. Avoid touching dropper tip to eye or other surfaces to prevent contamination. Wash hands before and after use.
Mechanism of Action
Blocks muscarinic acetylcholine receptors, leading to paralysis of iris sphincter muscle and ciliary body. In low doses for myopia, the exact mechanism is not fully understood but is thought to involve scleral remodeling and modulating retinal signaling pathways.
Pharmacokinetics
Onset
Mydriasis: 30-40 minutes; Cycloplegia: 1-3 hours (for higher concentrations, less pronounced with 0.01%). Myopia control effect is long-term.
Excretion
Mainly renal excretion of unchanged drug and metabolites.
Half life
Approximately 2-4 hours, but ocular effects can persist for days depending on concentration.
Absorption
Systemic absorption can occur from the conjunctival sac, though minimal with 0.01% strength.
Metabolism
Hepatic metabolism to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to atropine or any component of the formulation
- Narrow-angle glaucoma or predisposition to narrow-angle glaucoma
- Synechiae (adhesions) between iris and lens
Drug Interactions
Systemic Anticholinergics
May have additive systemic anticholinergic effects, increasing the risk of adverse reactions.
Other Mydriatics/Cycloplegics
Concurrent use may enhance mydriatic/cycloplegic effects.
Storage
Store at room temperature (20-25°C or 68-77°F). Protect from light. Do not freeze.
Overdose
Systemic overdose symptoms include tachycardia, fever, dry mouth, blurred vision, flushed skin, CNS excitation (confusion, hallucinations) especially in children. Treatment is supportive; physostigmine may be used as an antidote in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Atropine is excreted in breast milk; caution is advised when administered to a nursing mother.

Lab Monitoring
- Regular ophthalmic examinations including intraocular pressure monitoring for individuals predisposed to glaucoma.
Doctor Notes
- Educate parents/guardians on proper administration technique and potential side effects, especially systemic ones in young children.
- Monitor patient for potential increase in intraocular pressure and adverse reactions during treatment.
- Regular follow-up appointments are crucial to assess myopia progression and treatment efficacy.
Patient Guidelines
- Do not drive or operate machinery until vision clears and photophobia subsides.
- Wear sunglasses to reduce discomfort from light sensitivity.
- Do not use longer than prescribed.
- Inform your doctor about any unusual or severe side effects.
Missed Dose Advice
If a dose is missed, apply it as soon as remembered unless it is almost time for the next dose. Do not double the dose.
Driving Precautions
May cause blurred vision and increased sensitivity to light, which can impair the ability to drive or operate machinery. Exercise caution.
Lifestyle Advice
- Regular follow-up with an ophthalmologist for myopia progression assessment.
- Ensure proper lighting for reading and screen time to minimize eye strain.
