Description
Overview of the medicine
Escitalopram is an antidepressant used to treat major depressive disorder, generalized anxiety disorder, panic disorder, social anxiety disorder, and obsessive-compulsive disorder.
Uses & Indications
Dosage
Adults
Initial 10 mg once daily, may be increased to 20 mg/day after at least one week. Maximum 20 mg/day.
Elderly
Initial 5 mg once daily, maximum 10 mg/day.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. Use with caution in severe impairment.
How to Take
Oral, once daily, with or without food.
Mechanism of Action
It selectively inhibits the reuptake of serotonin (5-HT) in the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft and enhancing its neurotransmission.

Onset
Initial effects may be seen in 1-2 weeks, full therapeutic effect in 4-6 weeks.
Excretion
Excreted renally and hepatically.
Half life
Approximately 27-32 hours.
Absorption
Well absorbed orally, not significantly affected by food. Peak plasma levels usually in 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2C19, to inactive metabolites.
Side Effects
Contraindications
- Concomitant use with MAOIs (Monoamine Oxidase Inhibitors)
- Known hypersensitivity to escitalopram or citalopram
Drug Interactions
MAOIs
Increased risk of serotonin syndrome.
Warfarin
Increased risk of bleeding.
Other SSRIs, TCAs, Triptans, Fentanyl, Lithium, St. John's Wort
Increased risk of serotonin syndrome.
Storage
Store below 30°C, protected from light and moisture.
Overdose
Symptoms include dizziness, tremor, nausea, vomiting, somnolence, tachycardia, ECG changes. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to fetus. Excreted in breast milk, caution advised.
