Description
Overview of the medicine
Balo 10 mg Tablet contains Balosartan, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
The usual starting dose is 10 mg once daily. The dose may be increased to 20 mg once daily depending on blood pressure response.
Elderly
No initial dose adjustment is required for elderly patients, but renal function should be monitored.
Renal_impairment
No initial dose adjustment is generally required for mild to moderate renal impairment. For severe renal impairment (CrCl <30 mL/min), a lower starting dose (e.g., 5 mg) may be considered, and careful monitoring is advised.
How to Take
Take the tablet orally, once daily, with or without food. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Balosartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This inhibition leads to decreased vasoconstriction, reduced aldosterone secretion, and ultimately, lower blood pressure.
Pharmacokinetics
Onset
Blood pressure lowering effect starts within 2 hours, with maximal effect seen within 4-6 weeks of therapy.
Excretion
Excreted mainly unchanged in feces (around 80%) and urine (around 20%).
Half life
Approximately 9-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Undergoes minimal hepatic metabolism, primarily via glucuronidation.
Side Effects
Contraindications
- Pregnancy (especially in the second and third trimesters)
- Hypersensitivity to Balosartan or any component of the tablet
- Concomitant use with Aliskiren in patients with diabetes or renal impairment (CrCl <60 mL/min)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and worsen renal function in susceptible patients.
Diuretics (e.g., Furosemide, Hydrochlorothiazide)
May enhance the hypotensive effect, increasing the risk of symptomatic hypotension.
Potassium-sparing diuretics (e.g., Spironolactone, Amiloride) or potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose typically include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) may also occur. Treatment is supportive and symptomatic, including intravenous fluids to manage hypotension.
Pregnancy & Lactation
Balosartan is contraindicated during pregnancy, especially in the second and third trimesters, due to the risk of fetal harm. It is not recommended during breastfeeding as it may pass into breast milk; consult a doctor.

Lab Monitoring
- Serum creatinine and blood urea nitrogen (BUN) to monitor renal function
- Serum potassium levels, especially in patients with renal impairment or those taking potassium-sparing diuretics
- Blood pressure monitoring regularly
Doctor Notes
- Assess renal function and serum potassium before initiating treatment and periodically thereafter.
- Advise patients on lifestyle modifications to complement pharmacotherapy.
- Caution against concomitant use with NSAIDs due to potential for reduced efficacy and renal impairment.
