Description
Overview of the medicine
Betabis-Plus is a combination antihypertensive medicine containing a beta-blocker (Bisoprolol) and a thiazide diuretic (Hydrochlorothiazide) used for the management of essential hypertension.
Uses & Indications
Dosage
Adults
Usually one tablet (Bisoprolol 5 mg/HCTZ 6.25 mg) once daily, adjusted based on response. Max 10 mg Bisoprolol/12.5 mg HCTZ daily.
Elderly
Initial dose may be lower, titrate carefully.
Renal_impairment
Use with caution. Not recommended if CrCl <30 mL/min.
How to Take
Oral. Take with or without food, preferably in the morning.
Mechanism of Action
Bisoprolol selectively blocks beta-1 adrenergic receptors, reducing heart rate and cardiac output. Hydrochlorothiazide is a thiazide diuretic that inhibits sodium reabsorption in the distal convoluted tubule, increasing water and sodium excretion.

Onset
Bisoprolol: 2-4 hours, HCTZ: 2 hours.
Excretion
Bisoprolol: Renal (50% unchanged), Hepatic (metabolites). HCTZ: Renal (unchanged).
Half life
Bisoprolol: 10-12 hours, HCTZ: 5.6-14.8 hours.
Absorption
Bisoprolol is well absorbed (approx. 90%), HCTZ is absorbed (approx. 60-80%).
Metabolism
Bisoprolol: Hepatic (approx. 50%), HCTZ: Not significantly metabolized.
Side Effects
Contraindications
- Acute heart failure, cardiogenic shock
- Severe bradycardia, sick sinus syndrome, second or third-degree AV block (without pacemaker)
- Severe asthma or severe chronic obstructive pulmonary disease (COPD)
- Severe peripheral arterial occlusive disease
- Untreated pheochromocytoma
- Anuria, severe renal impairment (CrCl <30 mL/min)
- Hypersensitivity to bisoprolol, hydrochlorothiazide, or sulfonamide derivatives
Drug Interactions
NSAIDs
May reduce antihypertensive effect of HCTZ
Lithium
Renal clearance of lithium reduced by HCTZ, increasing lithium levels
Digitalis Glycosides
Increased risk of bradycardia
Other Antihypertensives
Increased hypotensive effect
Insulin/Oral Hypoglycemics
Beta-blockers may mask symptoms of hypoglycemia and potentiate the effect
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptomatic and supportive treatment. Bisoprolol overdose may cause bradycardia, hypotension, bronchospasm. HCTZ overdose may cause electrolyte depletion, dehydration.
Pregnancy & Lactation
Category C for both components. Use only if potential benefit justifies potential risk to the fetus. Not recommended during breastfeeding due to excretion in breast milk.
