Description
Overview of the medicine
Bromhexine is a mucolytic agent used to thin and loosen mucus in the airways, making it easier to cough up. It is commonly used in conditions associated with excessive or thick mucus, helping to clear congestion and improve breathing.
Uses & Indications
Dosage
Adults
Oral: 8-16 mg (2-5 tablets of 3mg) three times daily, or as directed by physician.
Elderly
Same as adult dose, but caution advised for those with underlying renal or hepatic impairment.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment. Consult physician for guidance.
How to Take
Bromhexine 3 mg tablet should be taken orally with water. It can be taken with or without food. For optimal effect, maintain good hydration.
Mechanism of Action
Bromhexine acts by depolymerizing mucopolysaccharides, thereby reducing the viscosity of bronchial secretions and facilitating their removal. It also increases serous mucus production by glands in the respiratory tract, making the mucus less sticky and easier to expectorate.
Pharmacokinetics
Onset
Mucolytic effect usually begins within 30 minutes to 1 hour after oral administration.
Excretion
Mainly excreted renally (about 90%) in the form of metabolites. A small amount is excreted unchanged.
Half life
The terminal elimination half-life of bromhexine is approximately 6.5 hours, though for metabolites it can be longer (15-20 hours).
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within approximately 1 hour. Bioavailability is low (about 20-25%) due to extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via oxidation and demethylation, to various hydroxylated metabolites, including ambroxol (an active metabolite).
Side Effects
Contraindications
- Hypersensitivity to bromhexine or any component of the formulation.
- Patients with a history of peptic ulcer (use with caution as it may disrupt the gastric mucosal barrier).
Drug Interactions
Antitussives
Concomitant use with antitussives (cough suppressants) may lead to a dangerous buildup of secretions due to the inhibited cough reflex. Avoid co-administration.
Antibiotics (e.g., Amoxicillin, Erythromycin, Oxytetracycline)
May increase the penetration of antibiotics into bronchial secretions, enhancing their efficacy.
Storage
Store in a cool, dry place, below 30°C. Protect from direct light and moisture. Keep out of reach and sight of children.
Overdose
Overdose symptoms are generally mild and may include nausea, vomiting, diarrhea, and stomach pain. Treatment is symptomatic and supportive; no specific antidote is known. Gastric lavage may be considered in severe cases shortly after ingestion.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Bromhexine is excreted in breast milk, so caution should be exercised when administered to a nursing mother. Consult a doctor before use during pregnancy or lactation.

Clinical Trials
Bromhexine is a well-established drug with extensive clinical use and numerous studies confirming its mucolytic efficacy and safety profile. Further trials mainly focus on combination therapies or specific populations (e.g., pediatric, geriatric).
Lab Monitoring
- No specific lab monitoring is typically required for bromhexine therapy. However, liver and kidney function tests may be considered in patients with pre-existing impairment.
Doctor Notes
- Assess the underlying cause of excessive mucus and cough (e.g., infection, asthma). Bromhexine is a symptomatic treatment.
- Emphasize the importance of adequate fluid intake to patients, as hydration significantly enhances the mucolytic action.
- Counsel patients on rare but serious adverse effects such as severe skin reactions; advise immediate discontinuation and medical consultation if such reactions occur.
- Consider dose adjustment in patients with severe hepatic or renal impairment.
Patient Guidelines
- Drink plenty of fluids (water, juices) to help thin and loosen mucus, which enhances the effect of bromhexine.
- Do not exceed the recommended dose. Taking more than prescribed will not improve symptoms faster and may increase side effects.
- Consult your doctor if symptoms persist or worsen after a few days of treatment.
- Inform your doctor or pharmacist about all other medications (including over-the-counter drugs, herbal supplements) you are taking.
- Discontinue use and seek immediate medical attention if you experience severe skin reactions (e.g., widespread rash, blistering) or difficulty breathing.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Bromhexine is generally not expected to impair the ability to drive or operate machinery. However, if you experience dizziness or drowsiness (which are rare side effects), it is advisable to avoid these activities until you know how the medicine affects you.
Lifestyle Advice
- Maintain good hydration by drinking adequate fluids throughout the day.
- Avoid smoking and exposure to irritants (e.g., strong fumes, pollutants) that may worsen cough and mucus production.
- Consider using a humidifier in your living space to help keep airways moist.
