Description
Overview of the medicine
Calpin-plus 5 mg tablet is a combination medicine used to treat high blood pressure (hypertension) and angina pectoris. It combines Amlodipine, a calcium channel blocker, and Atenolol, a beta-blocker, to provide a synergistic effect in lowering blood pressure and reducing cardiac workload.
Uses & Indications
Dosage
Adults
The usual starting dose is one Calpin-plus 5 mg tablet once daily. The dose may be adjusted by a physician based on response.
Elderly
Elderly patients may require a lower starting dose and careful monitoring due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment is generally required for amlodipine. Atenolol dose should be adjusted in patients with severe renal impairment. Consult physician for specific recommendations.
How to Take
Take the tablet orally, with or without food. It is recommended to take it at the same time each day for better blood pressure control.
Mechanism of Action
Amlodipine relaxes and widens blood vessels, allowing blood to flow more easily, thereby lowering blood pressure. Atenolol works by blocking certain natural chemicals in your body, like epinephrine, that affect the heart and blood vessels. This effect lowers your heart rate, blood pressure, and strain on your heart.
Pharmacokinetics
Onset
Amlodipine: Gradual onset, full effect within 6-12 hours. Atenolol: Within 1 hour.
Excretion
Amlodipine: Primarily excreted in the urine as inactive metabolites. Atenolol: Approximately 50% of the absorbed dose is excreted unchanged in the urine.
Half life
Amlodipine: 30-50 hours. Atenolol: 6-7 hours.
Absorption
Amlodipine is well absorbed from the gastrointestinal tract, with peak plasma concentrations occurring 6-12 hours post-dose. Atenolol is incompletely absorbed from the gastrointestinal tract (50-60%), with peak plasma concentrations occurring 2-4 hours.
Metabolism
Amlodipine is extensively metabolized in the liver to inactive metabolites. Atenolol undergoes minimal hepatic metabolism.
Side Effects
Contraindications
- Cardiogenic shock
- Sick sinus syndrome (without a pacemaker)
- Second- or third-degree AV block (without a pacemaker)
- Severe bradycardia (heart rate < 45-50 bpm)
- Decompensated heart failure
- Severe hypotension
- Known hypersensitivity to amlodipine, atenolol, or any component of the formulation
Drug Interactions
Digoxin
Concomitant use with atenolol may increase the risk of bradycardia and AV block.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
May reduce the antihypertensive effects of beta-blockers and amlodipine.
Antidiabetic agents (Insulin, oral hypoglycemics)
Atenolol may mask symptoms of hypoglycemia (e.g., tachycardia) and prolong hypoglycemic episodes.
Diltiazem or Verapamil (other calcium channel blockers)
May potentiate the negative inotropic and chronotropic effects, increasing the risk of bradycardia and heart block.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose may include severe hypotension, bradycardia, cardiac failure, and bronchospasm. Management involves symptomatic and supportive treatment, including gastric lavage, activated charcoal, IV fluids for hypotension, atropine for bradycardia, and glucagon for cardiac failure.
Pregnancy & Lactation
Pregnancy Category C for Amlodipine and Category D for Atenolol (especially in 2nd and 3rd trimesters due to potential fetal harm). Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as both drugs are excreted in breast milk.
