Cardinex-8000 Anti-Xa Injection contains enoxaparin sodium, a low molecular weight heparin, used to prevent and treat blood clots in various medical conditions.
Uses & Indications
Dosage
Adults
For treatment of DVT/PE: 1 mg/kg (100 anti-Xa IU/kg) subcutaneously every 12 hours. For an 80 kg individual, this would be 80 mg (8000 anti-Xa IU) every 12 hours. Dosage for other indications may vary.
Elderly
No dose adjustment generally required unless severe renal impairment. Monitor closely for signs of bleeding.
Renal_impairment
Severe renal impairment (CrCl < 30 mL/min): Dose reduction typically required; for treatment, 1 mg/kg (100 anti-Xa IU/kg) once daily.
How to Take
Administer by subcutaneous injection into the anterolateral or posterolateral abdominal wall. Alternate injection sites. Do not inject intramuscularly. Do not mix with other injections.
Mechanism of Action
Potentiates the action of antithrombin III, leading to selective inhibition of Factor Xa and, to a lesser extent, Factor IIa (thrombin). This results in antithrombotic activity.

Onset
Approximately 1 hour (anti-Xa activity) following SC injection.
Excretion
Mainly renal, with 10-40% of anti-Xa activity excreted unchanged in urine.

About 4.5 to 7 hours following a single SC dose, longer in renal impairment.

Contraindications
- Active major bleeding

- Recent hemorrhagic stroke

Dextran
Increased risk of bleeding.
Oral Anticoagulants (e.g., Warfarin)

Storage

Overdose
Overdose may lead to hemorrhagic complications. Manage with protamine sulfate, which partially neutralizes the anti-Xa activity. The dosage of protamine depends on the enoxaparin dose and the time since administration. Consult emergency medical services immediately.
Pregnancy & Lactation
Pregnancy category B. Use with caution during pregnancy if clearly needed, as insufficient data exist regarding human pregnancy. Enoxaparin is excreted into breast milk in very small amounts and is generally considered compatible with breastfeeding, but consult a doctor.
