Description
Overview of the medicine
Cefdox 200 mg Tablet is a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including those of the respiratory tract, urinary tract, skin, and soft tissues. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically 100-200 mg every 12 hours, depending on the severity and type of infection. Duration usually 7-14 days.
Elderly
Similar to adult dosage, but caution and dosage adjustment may be needed based on renal function.
Renal_impairment
Dosage adjustment is necessary for patients with creatinine clearance less than 30 mL/min. For CrCl 10-29 mL/min, 100 mg every 24 hours. For CrCl <10 mL/min, 100 mg every 48 hours.
How to Take
Cefdox 200 mg Tablet should be taken orally, preferably with food, to enhance absorption. Do not crush or chew the tablet; swallow it whole.
Mechanism of Action
Cefdoxime, the active metabolite of Cefdoxime Proxetil, is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are typically reached within 2-3 hours after oral administration.
Excretion
Cefdoxime is primarily eliminated unchanged by renal excretion (glomerular filtration and tubular secretion), with approximately 80% to 90% of the dose excreted in the urine within 12 hours.
Half life
The plasma elimination half-life is approximately 2 to 3 hours.
Absorption
Cefdoxime proxetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefdoxime. Bioavailability is approximately 50% and is increased when administered with food.
Metabolism
Cefdoxime proxetil is an ester prodrug that is rapidly hydrolyzed to its active metabolite, cefdoxime. Minimal metabolism of cefdoxime occurs in the liver.
Side Effects
Contraindications
- Hypersensitivity to cefdoxime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to any beta-lactam antibiotic (e.g., penicillins).
Drug Interactions
Probenecid
Increases plasma concentrations of cefdoxime by decreasing its renal excretion.
Oral anticoagulants
Concurrent use with warfarin may increase INR, requiring monitoring.
Antacids/H2-receptor antagonists
May decrease the absorption of cefdoxime, leading to reduced efficacy. Should be administered at least 2 hours after antacids.
Nephrotoxic drugs (e.g., aminoglycosides)
Increased risk of nephrotoxicity with concomitant use.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, and diarrhea. In the event of a significant overdose, gastric lavage or induced emesis may be considered. Hemodialysis may be useful for removing cefdoxime from the body, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefdoxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.

Clinical Trials
Extensive clinical trials have established the efficacy and safety of cefdoxime proxetil in treating various bacterial infections. Ongoing research focuses on antibiotic resistance patterns and new formulations.
Lab Monitoring
- Renal function tests (e.g., creatinine clearance) in patients with pre-existing renal impairment or during prolonged therapy.
- Liver function tests (e.g., AST, ALT) for prolonged use or if hepatic dysfunction is suspected.
- Complete blood count (CBC) if prolonged use is anticipated.
Doctor Notes
- Emphasize the importance of culture and sensitivity testing where feasible to guide antibiotic selection.
- Counsel patients on completing the full course of therapy, even if symptoms improve, to prevent treatment failure and resistance.
- Monitor for signs of hypersensitivity reactions, especially in patients with a history of penicillin allergy.
Patient Guidelines
- Take the medicine for the full prescribed duration, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Take with food to improve absorption and reduce gastrointestinal upset.
- Inform your doctor about any allergies, especially to penicillins or other cephalosporins.
- Report any severe or persistent side effects to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
Driving Precautions
Cefdoxime may cause dizziness in some individuals. Patients should be advised to exercise caution when driving or operating machinery until they know how the medication affects them.
Lifestyle Advice
- Maintain adequate hydration by drinking plenty of fluids.
- Avoid alcohol during treatment, as it may exacerbate gastrointestinal side effects.
