Description
Overview of the medicine
Cefotime is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is administered via injection.
Uses & Indications
Dosage
Adults
Usual dose is 1-2 g IM/IV every 8-12 hours. In severe infections, up to 12 g/day in divided doses.
Elderly
Dosage adjustment may be necessary based on renal function and overall health status.
Renal_impairment
Dosage should be reduced based on creatinine clearance. For CrCl < 20 mL/min, a reduced dose is required (e.g., half the usual dose at the usual interval).
How to Take
Administer by deep intramuscular (IM) injection or slow intravenous (IV) injection/infusion. For IV administration, reconstitute the powder with sterile water for injection, and administer over 3-5 minutes or as an infusion over 20-60 minutes.
Mechanism of Action
Cefotime is a bactericidal agent that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), leading to the disruption of the cell wall and subsequent bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid onset, typically within minutes after IV administration.
Excretion
Primarily excreted unchanged in the urine via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapid and complete absorption after intramuscular (IM) or intravenous (IV) administration.
Metabolism
Minor hepatic metabolism to a less active metabolite.
Side Effects
Contraindications
- Known hypersensitivity to cefotime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reaction to any other beta-lactam antibiotics (e.g., penicillins).
Drug Interactions
Probenecid
Increases and prolongs cefotime serum concentrations by decreasing renal tubular secretion.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity.
Oral Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect by altering gut flora; monitor INR.
Storage
Store the dry powder below 25°C, protected from light and moisture. Do not freeze. Refer to product insert for specific storage instructions of reconstituted solution.
Overdose
Symptoms of overdose may include convulsions, encephalopathy. Treatment is symptomatic and supportive. Hemodialysis may be useful in removing cefotime from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. Use during pregnancy only if clearly needed. Cefotime is excreted in small amounts in breast milk; use with caution in breastfeeding mothers.

Clinical Trials
Cefotime is a well-established antibiotic. Most clinical trials validating its efficacy and safety were conducted in the 1980s and 1990s, demonstrating its effectiveness against various bacterial infections.
Lab Monitoring
- Complete Blood Count (CBC) with differential, especially during prolonged therapy.
- Renal function tests (serum creatinine, BUN) in patients with pre-existing renal impairment or during prolonged high-dose therapy.
- Liver function tests (AST, ALT) occasionally.
Doctor Notes
- Prior to initiating therapy with cefotime, inquire about previous hypersensitivity reactions to cephalosporins, penicillins, or other beta-lactam antibiotics.
- Adjust dosage in patients with renal impairment based on creatinine clearance to avoid accumulation and potential toxicity.
- Monitor for signs of superinfection or C. difficile-associated diarrhea, especially during prolonged treatment.
Patient Guidelines
- Complete the full course of treatment as prescribed by your doctor, even if you feel better.
- Report any severe or persistent side effects immediately to your healthcare provider.
- Do not share this medicine with others, even if they have similar symptoms.
Missed Dose Advice
If a dose is missed, administer it as soon as you remember. If it is almost time for the next scheduled dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Cefotime generally does not impair the ability to drive or operate machinery. However, if you experience dizziness or other side effects that affect your concentration, avoid these activities.
Lifestyle Advice
- Maintain adequate hydration during treatment.
- Avoid alcohol consumption while on antibiotics, as it may worsen side effects or reduce efficacy.
