Cefixime is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
250 mg orally twice daily or 400 mg once daily, depending on the infection severity and type. Duration usually 7-14 days.
Elderly
Similar to adults, but renal function should be assessed and dosage adjusted if necessary.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose 300 mg daily. For creatinine clearance <20 mL/min, dose 100-200 mg daily or 200 mg every other day.
How to Take
Administer orally with or without food. Complete the full prescribed course of treatment even if symptoms improve to prevent recurrence and resistance.
Mechanism of Action
Cefixime is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.

Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Primarily excreted unchanged in the urine (about 50%) and bile (about 10%) within 24 hours.

Plasma half-life is typically 3-4 hours.
Absorption
Oral bioavailability is 40-50%, absorption is not significantly affected by food.
Metabolism
Not extensively metabolized in the body; approximately 10-20% may undergo minor metabolism.
Side Effects
Contraindications
- Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime Cmax and AUC.
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations. Monitor carbamazepine levels.
Oral Anticoagulants (e.g., Warfarin)
Cefixime may increase the anticoagulant effect, leading to increased prothrombin time (PT) or INR. Monitor closely and adjust anticoagulant dose if necessary.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive.
Pregnancy & Lactation
Pregnancy Category B. Cefixime is generally considered safe during pregnancy, but use only if clearly needed and with doctor's advice. Small amounts are excreted in breast milk; caution is advised during breastfeeding.
