Cefusil-CV 250 mg Tablet is a combination antibiotic used to treat a wide range of bacterial infections. It contains Cefuroxime Axetil, a second-generation cephalosporin, and Clavulanic Acid, a beta-lactamase inhibitor, which protects Cefuroxime from degradation by bacterial enzymes.
Uses & Indications
Dosage
Adults
For most infections, 250 mg (Cefuroxime) twice daily. For more severe infections or certain conditions like bronchitis/pneumonia, 500 mg (Cefuroxime) twice daily. Take with food. Duration is typically 5-10 days depending on the infection.
Elderly
No specific dose adjustment is generally required unless there is significant renal impairment.
Renal_impairment
Dose adjustment required for severe renal impairment (CrCl < 30 mL/min). Consult physician for specific recommendations.
How to Take
Take orally with food. Food increases the absorption of Cefuroxime Axetil. Do not crush or chew the tablet; swallow it whole.
Mechanism of Action
Cefuroxime, a cephalosporin, inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. Clavulanic acid, a beta-lactamase inhibitor, inactivates a wide range of beta-lactamase enzymes produced by bacteria, thus extending the spectrum of activity of cefuroxime against many beta-lactamase-producing strains.
Pharmacokinetics
Onset
Antibacterial effect typically observed within 1-2 hours of administration.
Excretion
Cefuroxime is excreted unchanged primarily by glomerular filtration and tubular secretion via the kidneys. Clavulanic acid is primarily excreted renally, with approximately 25-45% excreted unchanged.
Half life
Cefuroxime: Approximately 1.2 to 1.5 hours. Clavulanic acid: Approximately 1 hour.
Absorption
Cefuroxime axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases in the intestinal mucosa and blood to cefuroxime. Absorption is increased when taken with food. Peak plasma concentrations are achieved approximately 2-3 hours after oral administration. Clavulanic acid is also well absorbed orally.
Metabolism
Cefuroxime is not extensively metabolized in the body. Clavulanic acid is extensively metabolized to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Cefuroxime, Clavulanic Acid, any other cephalosporin antibiotics, or to any of the excipients.
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, monobactams, carbapenems).
Drug Interactions
Probenecid
Concomitant use with probenecid increases the area under the serum concentration-time curve of cefuroxime and reduces its renal clearance. This can lead to increased and prolonged plasma levels of cefuroxime.
Oral Contraceptives
Antibiotics may decrease the effectiveness of oral contraceptives. Patients should be advised to use additional barrier contraception.
Anticoagulants (e.g., Warfarin)
May enhance the anticoagulant effect; INR monitoring is recommended.
Antacids/H2-receptor antagonists
Drugs that reduce gastric acidity may result in a lower bioavailability of Cefuroxime Axetil. May need to be taken at least 2 hours apart.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose of cephalosporins can cause cerebral irritation leading to convulsions. Management includes symptomatic and supportive measures. Hemodialysis and peritoneal dialysis may be effective in removing cefuroxime and clavulanic acid from the body.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefuroxime and clavulanic acid are excreted in human milk in small quantities; caution should be exercised when administered to a nursing woman.
