Cefpodoxime is a third-generation cephalosporin antibiotic used to treat a variety of bacterial infections, including respiratory tract infections, urinary tract infections, and skin infections.
Uses & Indications
Dosage
Adults
The usual adult dosage varies depending on the infection. For most infections (e.g., pharyngitis/tonsillitis), 100 mg every 12 hours for 5-10 days. For complicated infections, it may be 200 mg every 12 hours.
Elderly
No specific dose adjustment required for elderly patients with normal renal function. Adjust dose for renal impairment.
Renal_impairment
For creatinine clearance 10-30 mL/min, administer the usual dose every 24 hours. For <10 mL/min, administer the usual dose every 48 hours. Hemodialysis patients should receive the usual dose after each dialysis session.
How to Take
Take orally, preferably with food, to enhance absorption.
Mechanism of Action
Cefpodoxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Onset
Peak plasma concentrations typically occur 2-3 hours after administration.
Excretion
The majority of cefpodoxime is excreted unchanged in the urine, primarily via glomerular filtration and tubular secretion.
Half life
The elimination half-life is approximately 2 to 3 hours.
Absorption
Cefpodoxime proxetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed to the active metabolite cefpodoxime. Absorption is enhanced by food.
Metabolism
Cefpodoxime proxetil is a prodrug that is de-esterified in the gastrointestinal tract to its active form, cefpodoxime. The active drug itself undergoes minimal hepatic metabolism.
Side Effects
Contraindications
- Patients with known hypersensitivity to cefpodoxime, other cephalosporins, or any component of the formulation.
- Patients with a history of severe hypersensitivity reactions to penicillin (e.g., anaphylaxis).
Drug Interactions
Probenecid
Reduces the renal excretion of cefpodoxime, leading to increased and prolonged plasma concentrations.
Oral anticoagulants
May enhance the anticoagulant effect when co-administered; monitor INR/PT.
Antacids/H2-receptor antagonists/PPIs
Can decrease the absorption of cefpodoxime, leading to lower plasma concentrations. Administer cefpodoxime at least 2 hours before or after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of severe overdose, supportive and symptomatic treatment is recommended. Hemodialysis may be useful in removing cefpodoxime from the body, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use in pregnancy only if clearly needed. Cefpodoxime is excreted in human milk; use with caution in nursing mothers.
