Description
Overview of the medicine
Chemocain-A is a synthetic amide-type local anesthetic used for various procedures to temporarily block nerve impulses, providing pain relief. It acts by stabilizing the neuronal membrane and preventing the initiation and conduction of nerve impulses.
Uses & Indications
Dosage
Adults
Individualized based on procedure, site, depth of anesthesia required. Max single dose usually 200 mg (40 mL of 0.5% solution) without adrenaline, or 500 mg (100 mL of 0.5% solution) with adrenaline.
Elderly
Reduced dosage recommended, starting at the lower end of the adult range, due to decreased hepatic function.
Renal_impairment
No specific dose adjustment for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Administered via injection by trained medical professionals into the skin, tissue, or near nerves, depending on the desired anesthetic effect.
Mechanism of Action
Chemocain-A stabilizes the neuronal membrane by inhibiting the ionic fluxes required for the initiation and conduction of impulses, thereby localizing the anesthetic action.
Pharmacokinetics
Onset
2-5 minutes (epidural/infiltration)
Excretion
Mainly excreted in urine as metabolites, small amount as unchanged drug.
Half life
1.5-3 hours
Absorption
Rapidly absorbed from injection site, systemic absorption depends on vascularity of the tissue.
Metabolism
Primarily metabolized in the liver by microsomal enzymes.
Side Effects
Contraindications
- Known hypersensitivity to amide-type local anesthetics
- Severe heart block
- Hypovolemic shock
Drug Interactions
MAOIs/TCAs
Increased risk of hypertension if used with vasoconstrictors like epinephrine (if included in formulation).
Beta-blockers
May increase Chemocain-A levels due to reduced hepatic blood flow.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms include CNS excitation (seizures), followed by depression (respiratory arrest), and cardiovascular effects (hypotension, bradycardia, cardiac arrest). Management involves maintaining airway, ventilation, treating seizures, and supporting cardiovascular function.
Pregnancy & Lactation
Use during pregnancy only if clearly needed. Excreted in breast milk in small amounts; use with caution.
