Overview of the medicine
Cifibet 100 mg tablet contains Ciprofloxacin, a fluoroquinolone antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
For uncomplicated UTIs, 100-250 mg orally twice daily for 3-7 days. For other infections, 250-750 mg orally twice daily depending on the severity and type of infection.
Elderly
Dosage adjustment may be necessary based on renal function. Generally, lower doses are recommended.
Renal_impairment
Dosage reduction is required for patients with creatinine clearance < 50 mL/min. For CrCl 30-50 mL/min, reduce dose by 50%. For CrCl < 30 mL/min, administer standard dose once daily.
How to Take
Take the tablet orally with water, preferably on an empty stomach (1-2 hours before or after meals) for better absorption, but can be taken with food to minimize GI upset. Avoid taking with dairy products or antacids containing magnesium, aluminum, or calcium.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of DNA synthesis and bacterial cell death.
Pharmacokinetics
Onset
Within 1-2 hours for antibacterial effect.
Excretion
Primarily renal (40-50% as unchanged drug) and to a lesser extent, fecal (20-35%).
Half life
Approximately 3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with bioavailability of about 70-80%. Peak plasma concentrations are reached 1-2 hours post-dose.
Metabolism
Limited hepatic metabolism (10-20%) via CYP450 enzymes, resulting in four active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones
- Concurrent administration with Tizanidine
- History of tendinitis or tendon rupture associated with fluoroquinolone use
Drug Interactions
NSAIDs
Concurrent use may increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance anticoagulant effects of warfarin, increasing bleeding risk. Monitor INR frequently.
Methotrexate
Increases methotrexate toxicity by inhibiting renal tubular secretion.
Theophylline
Ciprofloxacin can increase theophylline plasma levels, leading to toxicity. Monitor theophylline levels.
Antacids/Dairy products
Reduce ciprofloxacin absorption. Separate administration by at least 2 hours before or 6 hours after.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include renal toxicity, crystalluria, reversible renal impairment, and CNS effects (seizures). Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered. Maintain adequate hydration. Hemodialysis and peritoneal dialysis are generally not effective.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Ciprofloxacin is excreted in breast milk; caution is advised or consider alternative medication due to potential risk of arthropathy and other serious adverse reactions in the infant.
