Description
Overview of the medicine
Cilnidipine 10 mg Tablet is an antihypertensive medication used to treat high blood pressure (hypertension). It belongs to a class of drugs called calcium channel blockers.
Uses & Indications
Dosage
Adults
Usually 5-10 mg orally once daily. May be increased to a maximum of 20 mg once daily if necessary.
Elderly
Initiate with a lower dose (e.g., 5 mg once daily) and adjust according to response and tolerability.
Renal_impairment
No specific dose adjustment for mild to moderate renal impairment. Use with caution in severe impairment; consider lower starting dose.
How to Take
Take orally, preferably in the morning, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Cilnidipine selectively blocks L-type and N-type voltage-dependent calcium channels present in vascular smooth muscle cells and nerve endings. This leads to peripheral vasodilation, reduced sympathetic nerve activity, and ultimately a decrease in blood pressure.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly excreted as metabolites in urine (approx. 60%) and feces (approx. 40%).
Half life
Approximately 6-8 hours (terminal half-life)
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration reached within 1.5-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to Cilnidipine or other dihydropyridine derivatives.
- Cardiogenic shock.
- Severe aortic stenosis.
- Unstable angina.
- Severe hepatic impairment.
Drug Interactions
Digoxin
No significant interaction reported, but monitor digoxin levels.
Other Antihypertensives
Concomitant use may lead to additive hypotensive effects.
CYP3A4 Inducers (e.g., Rifampicin, Carbamazepine)
May decrease Cilnidipine plasma concentrations, potentially reducing its efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase Cilnidipine plasma concentrations, leading to enhanced hypotensive effect. Dose adjustment may be necessary.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive vasodilation with marked hypotension and possibly reflex tachycardia. Management is supportive, including gastric lavage, activated charcoal, and intravenous fluids for hypotension. Close monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C. Not recommended during pregnancy or lactation unless the potential benefit outweighs the potential risk to the fetus or infant. Consult a doctor.
