Description
Overview of the medicine
Cilvas-10 mg tablet contains Cilnidipine, a dual L-type and N-type calcium channel blocker used for the treatment of essential hypertension. It works by relaxing blood vessels, which lowers blood pressure and improves blood flow.
Uses & Indications
Dosage
Adults
The usual adult dose is 5-10 mg orally once daily. The dose may be adjusted based on blood pressure response, up to a maximum of 20 mg once daily.
Elderly
A starting dose of 5 mg once daily is generally recommended for elderly patients, with careful titration.
Renal_impairment
No dose adjustment is generally required for patients with mild to moderate renal impairment. Caution and close monitoring are advised in severe renal impairment.
How to Take
Take the tablet orally with a glass of water, with or without food. It is recommended to take the medicine at the same time each day to maintain consistent blood levels.
Mechanism of Action
Cilnidipine blocks both L-type voltage-dependent calcium channels in vascular smooth muscle and N-type calcium channels in sympathetic nerve endings. Blocking L-type channels leads to vasodilation, while blocking N-type channels reduces sympathetic neurotransmitter release, contributing to overall blood pressure reduction and potential renoprotective effects.
Pharmacokinetics
Onset
Blood pressure lowering effect typically begins within 1-2 hours.
Excretion
Approximately 60% of the dose is excreted via feces and about 40% via urine, mainly as metabolites.
Half life
The elimination half-life is approximately 5-8 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in about 2-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 3A4 (CYP3A4) enzyme system.
Side Effects
Contraindications
- Hypersensitivity to cilnidipine or any other dihydropyridine derivatives.
- Cardiogenic shock.
- Severe aortic stenosis.
Drug Interactions
Other Antihypertensives
Concomitant use with other antihypertensive agents may result in an additive hypotensive effect.
Strong CYP3A4 inducers (e.g., Rifampicin, Carbamazepine, Phenytoin)
May decrease plasma concentrations of cilnidipine, reducing its efficacy.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May increase plasma concentrations of cilnidipine, leading to enhanced effects and potential adverse reactions.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension, dizziness, and reflex tachycardia. Management is supportive and symptomatic; lay the patient supine with legs elevated, administer intravenous fluids and vasopressors if needed to restore blood pressure. Gastric lavage may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during lactation due to potential excretion in breast milk.

Clinical Trials
Cilnidipine has undergone several clinical trials demonstrating its efficacy in lowering blood pressure and improving cardiovascular outcomes. Some studies suggest renoprotective effects and benefits in patients with sympathetic overactivity due to its dual L/N-type calcium channel blocking action.
Lab Monitoring
- Regular blood pressure monitoring.
- Periodic assessment of renal function (serum creatinine, BUN) and liver function tests (ALT, AST).
Doctor Notes
- Cilnidipine's unique dual L/N-type calcium channel blocking mechanism offers advantages, particularly in patients with hypertension complicated by renal dysfunction or high sympathetic tone.
- Advise patients about potential peripheral edema and counsel on lifestyle modifications.
Patient Guidelines
- Take Cilvas-10 mg tablet exactly as prescribed by your doctor.
- Do not stop taking this medicine suddenly without consulting your doctor, even if your blood pressure seems normal.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
- Report any new or worsening side effects to your healthcare provider promptly.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cilvas-10 mg tablet may cause dizziness or lightheadedness, especially when starting treatment or increasing the dose. If affected, avoid driving or operating heavy machinery.
Lifestyle Advice
