Description
Overview of the medicine
Claudinil 100 mg Tablet is a medication used to treat symptoms of Irritable Bowel Syndrome (IBS) such as abdominal pain, bloating, and changes in bowel habits. It works by modulating tight junction proteins (claudins) in the gut.
Uses & Indications
Dosage
Adults
100 mg orally once daily, preferably before a meal. May be increased to 100 mg twice daily if needed, based on physician's assessment.
Elderly
No specific dose adjustment required, but caution is advised due to potential age-related decline in renal or hepatic function.
Renal_impairment
Mild to moderate renal impairment: No dose adjustment needed. Severe renal impairment: Not recommended, or use with extreme caution.
How to Take
Take the tablet orally with water, preferably 30 minutes before a meal. Do not crush or chew the tablet.
Mechanism of Action
Claudinil selectively modulates claudin proteins, particularly claudin-2, in the intestinal epithelial tight junctions. This action helps to restore barrier function, reduce visceral hypersensitivity, and normalize intestinal permeability, leading to alleviation of IBS symptoms.
Pharmacokinetics
Onset
Within 2-4 hours, full therapeutic effect usually observed within 1-2 weeks of regular use.
Excretion
Excreted primarily through feces (70%) and urine (30%) as metabolites.
Half life
Elimination half-life is around 4-6 hours.
Absorption
Rapidly absorbed after oral administration, reaching peak plasma concentrations within 1-2 hours. Bioavailability is approximately 60%.
Metabolism
Primarily metabolized in the liver via CYP3A4 and UGT pathways, forming inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Claudinil or any component of the formulation
- Patients with known or suspected mechanical gastrointestinal obstruction
- Severe hepatic impairment
Drug Interactions
Warfarin
May increase INR due to CYP3A4 inhibition; monitor coagulation parameters closely.
Ketoconazole
Strong CYP3A4 inhibitor, may increase plasma levels of Claudinil; dose adjustment may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include severe gastrointestinal discomfort, diarrhea, and hypotension. Treatment is supportive and symptomatic; gastric lavage or activated charcoal may be considered in acute cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs risk. Excretion into breast milk is unknown; caution advised for breastfeeding mothers.
