Description
Overview of the medicine
Clavusef-250-mg-tablet is an antibiotic medication combining Cefuroxime Axetil, a cephalosporin antibiotic, and Clavulanic Acid, a beta-lactamase inhibitor. It is used to treat a wide range of bacterial infections susceptible to this combination.
Uses & Indications
Dosage
Adults
One tablet (250 mg Cefuroxime Axetil / 125 mg Clavulanic Acid) twice daily for 5-10 days, depending on the infection severity and type.
Elderly
No specific dosage adjustment needed unless severe renal impairment. Refer to renal impairment dosage.
Renal_impairment
Dosage adjustment required based on creatinine clearance; e.g., for CrCl 10-29 mL/min, administer every 24 hours; for CrCl <10 mL/min, every 48 hours.
How to Take
Orally, preferably with food for enhanced absorption and to minimize gastrointestinal upset. Should be taken at regular intervals.
Mechanism of Action
Cefuroxime Axetil works by inhibiting bacterial cell wall synthesis, leading to cell lysis and death. Clavulanic Acid is a beta-lactamase inhibitor that protects cefuroxime from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending its spectrum of activity.
Pharmacokinetics
Onset
Rapid, typically within 1-2 hours.
Excretion
Mainly excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Clavulanic acid metabolites are also renally excreted.
Half life
Cefuroxime: approximately 1.2-1.5 hours. Clavulanic acid: approximately 1 hour.
Absorption
Cefuroxime Axetil is absorbed from the gastrointestinal tract and rapidly hydrolyzed to cefuroxime. Absorption is enhanced with food. Clavulanic acid is well absorbed orally.
Metabolism
Cefuroxime is not significantly metabolized in the liver. Clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime, other cephalosporins, penicillins, or clavulanic acid.
- History of severe hypersensitivity reaction to any beta-lactam antibiotic.
Drug Interactions
Warfarin
May enhance anticoagulant effect (monitor INR).
Probenecid
May increase cefuroxime levels by reducing renal tubular secretion.
Oral Contraceptives
May reduce efficacy of oral contraceptives (advise additional contraception).
Antacids/H2 blockers/PPIs
May reduce bioavailability of cefuroxime axetil.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, diarrhea, and convulsions in severe cases. Management is supportive, including gastric lavage or activated charcoal, and hemodialysis may be considered in severe renal impairment.
Pregnancy & Lactation
Pregnancy: Category B. Use only if clearly needed. Cefuroxime crosses the placenta. Lactation: Excreted in small amounts in breast milk. Use with caution. Potential for sensitization, diarrhea, and candidiasis in the infant.
