Overview of the medicine
Clogfree 75 mg Tablet contains Clopidogrel, an antiplatelet medication used to prevent blood clots in patients with heart disease or those at risk of stroke or heart attack.
Uses & Indications
Dosage
Adults
For acute coronary syndrome: A single 300 mg or 600 mg loading dose, followed by 75 mg once daily. For history of MI, stroke, or PAD: 75 mg orally once daily.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole with a glass of water.
Mechanism of Action
Clopidogrel is a prodrug that, upon activation by CYP450 enzymes (mainly CYP2C19), irreversibly binds to the P2Y12 adenosine diphosphate (ADP) receptor on platelets. This binding prevents ADP from activating the glycoprotein IIb/IIIa complex, thereby inhibiting platelet aggregation and the formation of blood clots.
Pharmacokinetics
Onset
Antiplatelet effect within 2 hours; maximal effect within 3-7 days of daily dosing.
Excretion
Approximately 50% renal and 46% fecal excretion within 120 hours of administration.
Half life
Parent drug: approximately 8 hours; Active metabolite: approximately 30 minutes.
Absorption
Rapidly absorbed after oral administration; however, the extent of absorption is at least 50%. It is a prodrug.
Metabolism
Primarily hepatic by CYP450 enzymes (e.g., CYP2C19, CYP1A2, CYP2B6, CYP3A4) to an active metabolite and inactive carboxylic acid derivatives.
Side Effects
Contraindications
- Hypersensitivity to the active substance or any excipients.
- Active pathological bleeding, such as peptic ulcer or intracranial hemorrhage.
- Severe hepatic impairment.
Drug Interactions
Warfarin (or other oral anticoagulants)
Increased risk of bleeding due to additive anticoagulant effects. Co-administration should be undertaken with caution.
NSAIDs (Non-Steroidal Anti-inflammatory Drugs)
Increased risk of gastrointestinal bleeding. Concomitant use should be with caution.
Proton Pump Inhibitors (e.g., Omeprazole, Esomeprazole)
May reduce the antiplatelet effect of Clopidogrel by inhibiting CYP2C19. Pantoprazole and Lansoprazole have less impact. Avoid concomitant use of Omeprazole/Esomeprazole if possible.
SSRIs (Selective Serotonin Reuptake Inhibitors) and SNRIs (Serotonin Norepinephrine Reuptake Inhibitors)
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose of clopidogrel may lead to prolonged bleeding time and subsequent bleeding complications. Management involves symptomatic treatment and supportive measures. If prolonged bleeding is observed, platelet transfusion may be considered.
Pregnancy & Lactation
Pregnancy: Not recommended during pregnancy unless clearly necessary, as animal studies have shown potential harm. Lactation: It is unknown whether clopidogrel is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
