Cosmozole 40 mg Capsule contains Omeprazole, a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It helps to treat acid-related diseases of the stomach and intestine.
Uses & Indications
Dosage
Adults
Usually 20-40 mg once daily, depending on the indication. For severe GERD or Zollinger-Ellison syndrome, doses up to 120 mg/day may be used.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential age-related decrease in renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take orally, preferably in the morning before breakfast. Capsules should be swallowed whole with water; do not crush, chew, or open the capsule.
Mechanism of Action
Omeprazole works by irreversibly blocking the proton pump (H+/K+-ATPase) in the gastric parietal cells, thereby preventing the secretion of gastric acid into the stomach lumen.
Pharmacokinetics
Onset
Within 1 hour (initial effect), full effect after several days of treatment.
Excretion
Approximately 77% excreted in the urine as metabolites, and the remainder in feces.
Half life
Plasma half-life is approximately 0.5 to 1 hour, but its duration of action on acid secretion is much longer (up to 72 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5 to 3.5 hours. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme system, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Co-administration with nelfinavir.
Drug Interactions
Warfarin
Increased International Normalized Ratio (INR) and prothrombin time; monitor closely.
Clopidogrel
Reduced antiplatelet effect of clopidogrel. Avoid concomitant use if possible.
Methotrexate
Increased and prolonged methotrexate levels; consider temporary PPI discontinuation.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretroviral drugs, leading to loss of therapeutic effect.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, blurred vision, tachycardia, nausea, vomiting, sweating, headache, dry mouth, and other gastrointestinal disturbances. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
