Description
Overview of the medicine
Cytosor 100 mg Injection is an antineoplastic agent used in the treatment of various types of cancer, primarily targeting rapidly dividing cells. It works by interfering with DNA synthesis and repair.
Uses & Indications
Dosage
Adults
Dosage varies greatly depending on the specific cancer and treatment protocol. Typically administered intravenously (IV) at doses ranging from 100 mg/m² to 3000 mg/m² over several days, often in cycles.
Elderly
Careful dose adjustment may be required due to decreased renal function and potential for increased toxicity. Initial doses may be lower.
Renal_impairment
Dose reduction is necessary. Specific guidelines should be followed based on creatinine clearance.
How to Take
Administered intravenously (IV) as a bolus injection or short/continuous infusion, or subcutaneously (SC). Intrathecal administration is also possible for central nervous system involvement. Must be administered by qualified healthcare professionals.
Mechanism of Action
Cytosorbine is a pyrimidine analog that acts as an antimetabolite. It is phosphorylated intracellularly to an active triphosphate form, which then inhibits DNA polymerase, thereby interfering with DNA synthesis and repair. It also inhibits RNA synthesis to a lesser extent.
Pharmacokinetics
Onset
Rapid, within minutes of IV administration.
Excretion
Mainly renal, approximately 80% excreted in urine within 24 hours as inactive metabolites.
Half life
Biphasic elimination with initial half-life of 10-20 minutes and terminal half-life of 1-3 hours.
Absorption
Rapidly absorbed after intravenous administration, poor oral absorption.
Metabolism
Primarily hepatic, converted to inactive uracil derivative.
Side Effects
Contraindications
- Hypersensitivity to Cytosorbine or any component of the formulation.
- Pre-existing severe bone marrow depression (unless the benefit outweighs the risk).
- Severe hepatic or renal impairment.
Drug Interactions
Digoxin
May reduce the absorption of oral digoxin.
Gentamicin
Increased risk of neurotoxicity.
Myelosuppressive agents
Increased risk of severe myelosuppression.
Storage
Store reconstituted solution at room temperature (15-25°C) and use within 8 hours. Store unopened vials at controlled room temperature (20-25°C), protected from light.
Overdose
Overdose symptoms include severe myelosuppression, neurotoxicity, and gastrointestinal toxicity. Management is supportive, including blood transfusions, antibiotics for infection, and general supportive care. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category D. May cause fetal harm when administered to a pregnant woman. Should not be used during pregnancy unless the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, breastfeeding should be discontinued during therapy.

- High-dose Cytosorbine requires prophylactic steroid eye drops to prevent conjunctivitis and vigilant neurological monitoring.
- Discuss fertility preservation options with patients of childbearing potential prior to initiation of therapy.
Patient Guidelines
- Report any fever, unusual bleeding or bruising, or signs of infection immediately.
- Stay well-hydrated during treatment.
