Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg every 12 hours. For uncomplicated gonorrhea, a single dose of 400 mg is recommended.
Elderly
No specific dosage adjustment is necessary for elderly patients unless there is severe renal impairment.
Renal_impairment
In patients with creatinine clearance less than 60 mL/min, the dose should be reduced. For creatinine clearance between 21-60 mL/min, 300 mg daily; for less than 20 mL/min, 100-200 mg daily.
How to Take
D-Fix 400 mg capsule can be taken with or without food. It should be swallowed whole with a glass of water. It is important to complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, preventing the final transpeptidation step of peptidoglycan synthesis.

Antibacterial effect typically observed within 2-4 hours.
Excretion
Primarily renal excretion (approximately 50% of absorbed dose via urine) and biliary excretion (about 10% via bile).

Oral absorption of cefixime is about 30-50% and is relatively slow, reaching peak plasma concentrations 2-8 hours after administration.
Metabolism
Cefixime is not extensively metabolized; about 50% of an absorbed dose is excreted unchanged in the urine.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or other cephalosporin antibiotics.
- History of serious hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Probenecid
Increases cefixime plasma concentrations by inhibiting renal excretion.
Carbamazepine
Concomitant administration may increase carbamazepine plasma concentrations.
Oral Contraceptives
May decrease the effectiveness of oral contraceptives, requiring additional birth control methods.
Warfarin and other anticoagulants
May increase prothrombin time with or without bleeding. Close monitoring of INR is recommended.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution in nursing mothers.
