Description
Overview of the medicine
D-Rise 40000 IU Capsule contains Cholecalciferol (Vitamin D3), which is essential for calcium absorption, bone health, and immune function. It is used to treat and prevent Vitamin D deficiency and associated conditions.
Uses & Indications
Dosage
Adults
For severe deficiency: 40,000 IU once weekly for 7 weeks, followed by maintenance. For moderate deficiency: 40,000 IU once every 2-4 weeks or as directed by physician.
Elderly
Same as adult dosage, but caution with concomitant medication and renal function.
Renal_impairment
Use with caution. Monitoring of calcium and phosphate levels is crucial. Specific dosage adjustment may be required.
How to Take
Oral. The capsule should be swallowed whole with water, preferably with a main meal to enhance absorption.
Mechanism of Action
Cholecalciferol is metabolized in the liver to calcifediol (25-hydroxyvitamin D3) and then in the kidneys to calcitriol (1,25-dihydroxyvitamin D3), the active form of Vitamin D. Calcitriol regulates calcium and phosphate homeostasis by promoting their absorption from the gut, reabsorption in the kidneys, and mobilization from bone.
Pharmacokinetics
Onset
Several hours to days for biological effects to become apparent, full effect within weeks.
Excretion
Mainly excreted in bile and feces, with a small amount excreted in urine.
Half life
Circulating 25(OH)D3 has a half-life of 2-3 weeks, while the active 1,25(OH)2D3 has a half-life of 4-6 hours.
Absorption
Well absorbed from the small intestine, especially when taken with fatty meals.
Metabolism
Primarily hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol), then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol).
Side Effects
Contraindications
- Hypercalcaemia (high calcium levels)
- Hypervitaminosis D (Vitamin D toxicity)
- Nephrolithiasis (kidney stones) or nephrocalcinosis
- Hypersensitivity to cholecalciferol or any excipients
Drug Interactions
Glucocorticoids
May decrease the effects of Vitamin D.
Thiazide diuretics
May increase risk of hypercalcaemia by reducing urinary calcium excretion.
Cardiac glycosides (e.g., Digoxin)
Increased risk of arrhythmias if hypercalcaemia occurs.
Anticonvulsants (e.g., Phenytoin, Barbiturates)
May reduce Vitamin D levels by accelerating its metabolism.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include hypercalcaemia, nausea, vomiting, constipation, anorexia, polyuria, polydipsia, weakness, headache, and in severe cases, renal impairment, cardiac arrhythmias, and coma. Treatment involves stopping Vitamin D, hydration, and managing hypercalcaemia.
Pregnancy & Lactation
Generally considered safe at recommended daily allowances. High doses should be used with caution and only if clearly needed, under medical supervision, due to potential risk of hypercalcaemia in the mother and fetus.

- Serum phosphate levels
- Serum 25(OH)D levels (to monitor Vitamin D status)
- Renal function (e.g., creatinine)
Doctor Notes
- Advise patients on dietary sources of Vitamin D and safe sun exposure.
- Emphasize the importance of adherence to prescribed dosage and regular follow-up for monitoring serum levels.
- Consider co-administering calcium if dietary intake is insufficient.
Patient Guidelines
- Take as directed by your doctor.
- Do not exceed the recommended dose.
- Take with food to improve absorption.
- Report any symptoms of hypercalcaemia immediately (e.g., severe nausea, vomiting, excessive thirst, frequent urination).
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
D-Rise 40000 IU Capsule is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Maintain a balanced diet.
