Description
Overview of the medicine
Depresil 0.5 mg tablet is an antidepressant medication used to treat major depressive disorder, generalized anxiety disorder, and panic disorder. It helps restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Initial dose 0.5 mg once daily, may be increased to a maximum of 2 mg daily based on clinical response and tolerability. Maintenance dose typically 0.5-1 mg daily.
Elderly
A lower initial dose of 0.25 mg once daily may be considered, with careful titration up to a maximum of 1 mg daily.
Renal_impairment
No specific dose adjustment is typically required for mild to moderate renal impairment. For severe impairment (CrCl <30 mL/min), caution is advised, and a lower dose may be necessary.
How to Take
Depresil tablets should be taken orally once daily, with or without food. It is recommended to take the tablet at the same time each day.
Mechanism of Action
Depresil works by selectively inhibiting the reuptake of serotonin in the brain's neurons. This increases the concentration of serotonin in the synaptic cleft, thereby enhancing serotonergic neurotransmission and improving mood and anxiety symptoms.
Pharmacokinetics
Onset
Initial therapeutic effects may be observed within 1-2 weeks, full effect in 4-6 weeks.
Excretion
Excreted mainly via urine as metabolites, with a small portion excreted in feces.
Half life
Approximately 20-30 hours, allowing for once-daily dosing.
Absorption
Well absorbed orally, with peak plasma concentrations achieved within 2-4 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes, mainly CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Serotalin Tartrate or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy.
- Severe hepatic impairment.
Drug Interactions
MAOIs
Concurrent use is contraindicated due to risk of serotonin syndrome.
NSAIDs
Increased risk of gastrointestinal bleeding.
Warfarin
May increase anticoagulant effect, monitor INR.
Tricyclic Antidepressants (TCAs)
May increase plasma levels of TCAs, requiring dose adjustment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, tachycardia, tremor, and dizziness. Severe overdose can lead to serotonin syndrome, seizures, and coma. Treatment is symptomatic and supportive. Maintain an open airway, monitor cardiac and vital signs, and administer activated charcoal if ingestion is recent.
Pregnancy & Lactation
Pregnancy Category C. Depresil should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; therefore, caution should be exercised when administered to a nursing woman.
