Desofast 2.5 mg/5 mL Syrup is an antihistamine used to relieve symptoms of allergic rhinitis and chronic idiopathic urticaria. It is a non-drowsy formulation.
Uses & Indications
Dosage
Adults
5 mg (10 mL syrup) once daily.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with renal or hepatic impairment.
Renal_impairment
For patients with renal impairment, the starting dose should be 5 mg (10 mL syrup) every other day. Consult a physician.
Pediatric_1_5_years
1.25 mg (2.5 mL syrup) once daily.
Pediatric_6_11_years
2.5 mg (5 mL syrup) once daily.
Pediatric_6_11_months
1.25 mg (2.5 mL syrup) once daily.
How to Take
Take orally, with or without food. Use an accurate measuring device (e.g., spoon or syringe) provided with the syrup to ensure correct dosage.
Mechanism of Action
Desloratadine is a selective peripheral H1-receptor antagonist. It inhibits the release of histamine from mast cells and basophils, and also inhibits the expression of adhesion molecules and pro-inflammatory cytokines, thus reducing allergic symptoms.

Onset
Onset of action typically occurs within 1 hour.
Excretion
Approximately 87% of the desloratadine dose is excreted in urine and feces as metabolites.
Half life
Mean terminal elimination half-life is approximately 27 hours.
Absorption
Rapidly absorbed after oral administration, detectable in plasma within 30 minutes. Peak plasma concentrations (Cmax) are reached approximately 3 hours after dosing.
Metabolism
Extensively metabolized to an active metabolite, 3-hydroxydesloratadine. Metabolism is primarily by hydroxylation, followed by glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to desloratadine, loratadine, or any component of the formulation.
- Severe hepatic impairment.
Drug Interactions
Cimetidine
Increases plasma concentration of desloratadine, but no clinically significant adverse effects observed.
Erythromycin
Increases plasma concentration of desloratadine, but no clinically significant adverse effects observed.
Ketoconazole
Increases plasma concentration of desloratadine, but no clinically significant adverse effects observed.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Symptoms of overdose may include somnolence, increased heart rate, and headache. In the event of an overdose, symptomatic and supportive treatment is recommended. Desloratadine is not eliminated by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if potential benefit justifies the potential risk to the fetus. Desloratadine is excreted in breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
