Description
Overview of the medicine
Dexagen-L eye drop is a combination medication containing Dexamethasone, a corticosteroid, and Levofloxacin, a fluoroquinolone antibiotic. It is used to treat inflammatory conditions of the eye where there is a risk of bacterial infection or an existing bacterial infection.
Uses & Indications
Dosage
Adults
1-2 drops in the affected eye(s) 4 times daily, or as directed by the physician. Dosage may be tapered as symptoms improve.
Elderly
No specific dosage adjustment required, but use with caution due to potential for systemic side effects with prolonged use.
Renal_impairment
No specific dosage adjustment required for topical ophthalmic use.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to form a pouch, instill the drop(s) into the pouch. Close eye gently and apply pressure to the inner corner of the eye for 1-2 minutes to prevent systemic absorption. Do not touch the dropper tip to any surface.
Mechanism of Action
Dexamethasone reduces inflammation by inhibiting the release of inflammatory mediators and suppressing immune response. Levofloxacin kills bacteria by inhibiting DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination.

Onset
Anti-inflammatory effects within hours; antibacterial effects can be seen within hours of initial dose.
Excretion
Primarily renal for systemically absorbed components; most of the topically applied drug is cleared locally or by tears.
Half life
Local half-life in ocular tissues varies, systemic half-life of absorbed components is longer but systemic levels are very low.
Absorption
Minimal systemic absorption after ocular administration. Local absorption into ocular tissues.
Metabolism
Primarily metabolized in the liver for systemically absorbed components; local metabolism in ocular tissues is minimal.
Side Effects
Contraindications
- Viral diseases of the cornea and conjunctiva (e.g., Herpes Simplex Keratitis, Vaccinia, Varicella)
- Mycobacterial infection of the eye
- Fungal diseases of ocular structures
- Hypersensitivity to Dexamethasone, Levofloxacin, or any components of the formulation
Drug Interactions
Systemic corticosteroids
Prolonged concomitant use might increase systemic absorption, though unlikely with typical ophthalmic use.
Other topical ophthalmic medications
If using other eye drops, wait at least 5-10 minutes between applications to prevent wash-out.
Storage
Store in a cool, dry place, below 25°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
An acute overdose is unlikely with ophthalmic administration. If accidental ingestion occurs, seek medical advice. Management is supportive.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when administered to a nursing mother, as small amounts may be excreted in breast milk. Consult a doctor.
