Dextroamphetamine is a central nervous system stimulant used to treat attention deficit hyperactivity disorder (ADHD) and narcolepsy. It works by increasing the levels of certain neurotransmitters in the brain.
Uses & Indications
Dosage
Adults
For ADHD: Typically 20-60 mg once daily in the morning for extended-release capsules. Dosage should be individualized and titrated. For Narcolepsy: 5-60 mg daily in divided doses.
Elderly
Initiate with lower doses and monitor closely for side effects due to increased sensitivity.
Renal_impairment
Use with caution. Dose adjustments may be necessary depending on the degree of impairment.
How to Take
Take orally, usually once daily in the morning, with or without food. Capsules should be swallowed whole; do not chew or crush.
Mechanism of Action
Dextroamphetamine works by blocking the reuptake of norepinephrine and dopamine into the presynaptic neuron and increasing the release of these monoamines into the extraneuronal space.

Onset
30-60 minutes for immediate release; longer for extended release.
Excretion
Primarily renal. Excretion is pH-dependent; increased in acidic urine, decreased in alkaline urine.
Half life
Approximately 10-12 hours in adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached within 1-3 hours for immediate release. Extended-release forms have a slower absorption with a biphasic peak.
Metabolism
Primarily hepatic, via hydroxylation and deamination.

Storage

Overdose
Symptoms include restlessness, tremor, hyperreflexia, rapid respiration, confusion, assaultiveness, hallucinations, panic states, hyperpyrexia, rhabdomyolysis, and convulsions. Management is primarily supportive.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during lactation as it is excreted in breast milk.
