Description
Overview of the medicine
Dextromethorphan is a cough suppressant used for the temporary relief of coughs caused by minor throat and bronchial irritation, such as from the common cold or flu. It works by affecting the cough reflex in the brain.
Uses & Indications
Dosage
Adults
10-20 mg every 4-6 hours, or 30 mg every 6-8 hours, not to exceed 120 mg in 24 hours.
Elderly
Similar to adult dosage; caution advised for those with comorbidities or polypharmacy.
Renal_impairment
Use with caution; dose reduction may be necessary in severe renal impairment. Consult physician.
How to Take
Administer orally. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla oblongata to elevate the cough threshold, thereby suppressing the cough reflex. It is a non-opioid antitussive.
Pharmacokinetics
Onset
15-30 minutes
Excretion
Mainly excreted in the urine as unchanged drug and metabolites.
Half life
2-4 hours (variable due to CYP2D6 polymorphism)
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6, to its active metabolite, dextrorphan.
Side Effects
Contraindications
- Hypersensitivity to dextromethorphan or any component of the formulation
- Concurrent use with or within 14 days of discontinuing monoamine oxidase inhibitors (MAOIs)
- Asthma exacerbation or severe respiratory depression
Drug Interactions
SSRIs and TCAs
Increased risk of serotonin syndrome when co-administered.
Alcohol and other CNS depressants
May enhance central nervous system (CNS) depression, leading to increased drowsiness or dizziness.
Monoamine Oxidase Inhibitors (MAOIs)
Concurrent use may lead to serotonin syndrome, characterized by hyperthermia, rigidity, mental status changes, and autonomic instability.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine)
May increase plasma concentrations of dextromethorphan, leading to exaggerated effects and toxicity.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, drowsiness, blurred vision, ataxia, nystagmus, CNS excitation, and respiratory depression. Severe cases may lead to serotonin syndrome, hallucinations, and coma. Treatment is symptomatic and supportive. Naloxone may be used to reverse CNS and respiratory depression, though dextromethorphan is not an opioid.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and potential benefits outweigh risks. Consult a doctor before use during pregnancy or lactation.
