Description
Overview of the medicine
Dianide 100 mg suspension is an antibiotic used to treat various bacterial and parasitic infections. It contains Metronidazole, which works by stopping the growth of certain bacteria and parasites.
Uses & Indications
Dosage
Adults
Not typically used in 100 mg suspension form for adults. For amoebiasis, adults usually take 400-800 mg three times daily (as tablets).
Elderly
Similar to adults, with caution in hepatic impairment.
Renal_impairment
Dosage adjustment not usually required for mild to moderate impairment. In severe impairment (CrCl < 10 mL/min), reduce dose by 50% or prolong dosing interval.
How to Take
Shake well before use. Administer orally, usually with or after food to minimize GI upset.
Mechanism of Action
Metronidazole is a nitroimidazole antimicrobial. It enters the organism, and in anaerobic conditions, its nitro group is reduced by electron transport proteins to reactive intermediates. These intermediates bind to and damage DNA, proteins, and cell membranes, leading to cell death.
Pharmacokinetics
Onset
Typically within 1-2 hours after oral administration.
Excretion
Mainly renal (60-80% as metabolites and unchanged drug), some fecal excretion (6-15%).
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Well absorbed orally, with peak plasma concentrations occurring within 1-2 hours.
Metabolism
Primarily hepatic, via oxidation and glucuronidation, forming hydroxylated and carboxylic acid metabolites.
Side Effects
Contraindications
- Hypersensitivity to metronidazole or other nitroimidazole derivatives
- First trimester of pregnancy (relative contraindication)
- Active central nervous system disease
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia). Avoid alcohol.
Lithium
Increases plasma lithium levels, risk of lithium toxicity.
Warfarin
Potentiates anticoagulant effect, monitor INR.
Disulfiram
Psychotic reactions, avoid concomitant use.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, and mild disorientation. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Generally avoided in the first trimester of pregnancy; use only if clearly needed in subsequent trimesters. Metronidazole passes into breast milk; consider temporary discontinuation of breastfeeding or alternative treatment.
