Overview of the medicine
Diclofenac (Timed Release) 100 mg capsule is a non-steroidal anti-inflammatory drug (NSAID) used for the treatment of pain and inflammation associated with various conditions such as osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, and acute musculoskeletal disorders.
Uses & Indications
Dosage
Adults
Usually 100 mg once daily, preferably with food. Max 100 mg/day.
Elderly
Lower doses may be required, monitor for adverse effects. Usually 100 mg once daily.
Renal_impairment
Use with caution; dose adjustment may be necessary for severe impairment. Avoid if GFR < 30 mL/min.
How to Take
Take the capsule whole with a glass of water, preferably with or after food to minimize gastrointestinal upset. Do not chew or crush.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by blocking cyclooxygenase (COX-1 and COX-2) enzymes, thereby reducing inflammation, pain, and fever.

Onset
Approximately 1-2 hours (for initial release), sustained effect over 12-24 hours.
Excretion
Primarily excreted via urine (60%) as metabolites and bile (35%) as metabolites.
Half life
About 1-2 hours (parent compound), but active metabolites contribute to prolonged effect. Timed-release extends effective half-life.
Absorption
Well absorbed orally. Timed-release formulation ensures slower and sustained absorption.
Metabolism
Extensively metabolized in the liver (CYP2C9, CYP2C19, CYP3A4) mainly via hydroxylation and glucuronidation.

- Third trimester of pregnancy

Drug Interactions
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration/bleeding.
Lithium/Digoxin
Increased plasma levels of lithium/digoxin.
Warfarin/Anticoagulants
Increased risk of bleeding.
Diuretics/ACE inhibitors
Reduced antihypertensive effect; increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include lethargy, drowsiness, nausea, vomiting, epigastric pain, GI bleeding. Management is symptomatic and supportive; gastric lavage or activated charcoal may be considered within 1 hour of ingestion. No specific antidote.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to risk of premature closure of ductus arteriosus. Use with caution in first and second trimesters only if clearly needed. Excreted in breast milk in small amounts; use with caution.
