Description
Overview of the medicine
Diloren 1% w/w Gel contains Diclofenac Sodium, a non-steroidal anti-inflammatory drug (NSAID), used for topical relief of pain and inflammation associated with musculoskeletal conditions.
Uses & Indications
Dosage
Adults
Apply 2 to 4 grams of gel (approx. 2.0 to 2.5 cm long strip) to the affected area 2 to 4 times a day, rubbing it in gently. The amount of gel depends on the size of the affected area.
Elderly
The usual adult dosage applies. No special dose adjustment is required.
Renal_impairment
No specific dose adjustment is required for topical application due to minimal systemic absorption.
How to Take
Apply a small amount of gel thinly to the affected skin area and rub it gently into the skin. Wash hands thoroughly after application, unless hands are the treated area.
Mechanism of Action
Diclofenac inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes. This reduces inflammation, pain, and fever.
Pharmacokinetics
Onset
Onset of pain relief can occur within 30 minutes to an hour after application.
Excretion
Metabolites are primarily excreted in the urine (around 60%) and bile (around 35%).
Half life
Systemic plasma half-life is approximately 1-2 hours, but it accumulates in synovial fluid, prolonging local action.
Absorption
Approximately 6% of the applied diclofenac is absorbed systemically after topical application. Absorption is higher in areas with compromised skin barrier.
Metabolism
The small portion absorbed systemically is metabolized in the liver, primarily via hydroxylation and glucuronidation.
Side Effects
Contraindications
- Hypersensitivity to diclofenac, aspirin, or other non-steroidal anti-inflammatory drugs (NSAIDs).
- History of asthma, urticaria, or acute rhinitis precipitated by aspirin or other NSAIDs.
- Third trimester of pregnancy due to risk of premature closure of the ductus arteriosus.
- Application to open wounds, infected skin, or mucous membranes.
Drug Interactions
Oral NSAIDs
Concomitant use with oral NSAIDs may lead to an increased incidence of systemic side effects.
Anticoagulants
Theoretical risk of increased bleeding tendency with concomitant use, though systemic absorption is minimal.
Storage
Store below 30°C. Protect from direct sunlight and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with topical application due to minimal systemic absorption. If accidentally ingested, symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Avoid during the third trimester of pregnancy. Use with caution during the first and second trimesters and during lactation, only if the potential benefits outweigh the risks and under medical advice. Minimal systemic absorption, but potential risks to the fetus/infant should be considered.
