Overview of the medicine
Diobis 10 mg Tablet contains Bisoprolol Fumarate, a selective beta-1 adrenergic blocking agent, used primarily for the management of hypertension, angina pectoris, and chronic heart failure.
Uses & Indications
Dosage
Adults
Hypertension/Angina: Initially 5 mg once daily, maximum 20 mg once daily. Chronic Heart Failure: Initially 1.25 mg once daily, gradually titrated up to a maximum of 10 mg once daily.
Elderly
No dosage adjustment is generally required, but close monitoring is advised.
Renal_impairment
For severe renal impairment (creatinine clearance < 20 mL/min), initial dose of 2.5 mg once daily, maximum 10 mg once daily.
How to Take
Take the tablet orally once daily in the morning, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Bisoprolol selectively blocks beta-1 adrenergic receptors, primarily in the heart. This reduces heart rate, myocardial contractility, and cardiac output, leading to a decrease in blood pressure. It also inhibits the release of renin.

Side Effects
Contraindications
- Acute heart failure or decompensated heart failure requiring intravenous inotropic therapy
- Cardiogenic shock
- Second or third degree AV block without a pacemaker
- Sick sinus syndrome
- Sinoatrial block
- Bradycardia (heart rate < 60 beats/min before therapy)
- Severe asthma or severe chronic obstructive pulmonary disease
- Severe peripheral arterial occlusive disease or Raynaud's syndrome
- Pheochromocytoma (untreated)
- Metabolic acidosis
- Hypersensitivity to Bisoprolol or any excipients
Drug Interactions
NSAIDs
May reduce the hypotensive effect of bisoprolol.
Clonidine
Risk of rebound hypertension upon clonidine withdrawal if beta-blocker is not withdrawn first.
Insulin and oral antidiabetics
May mask symptoms of hypoglycemia and potentiate hypoglycemic effects.
Antiarrhythmics (e.g., disopyramide, amiodarone)
Increased risk of bradycardia and cardiac depression.
Calcium channel blockers (e.g., verapamil, diltiazem)
May increase risk of bradycardia, AV block, and heart failure.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency. Management involves supportive and symptomatic treatment, including atropine for severe bradycardia, IV fluids and vasopressors for hypotension, and bronchodilators for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Bisoprolol is excreted in breast milk; advise caution in nursing mothers. Consult a doctor.
