Description
Overview of the medicine
Dobutamine is a synthetic catecholamine used for the short-term treatment of cardiac decompensation due to depressed contractility, resulting from organic heart disease or cardiac surgical procedures.
Uses & Indications
Dosage
Adults
Usually 2.5-20 mcg/kg/min by continuous intravenous infusion. Dose should be titrated based on patient's response and hemodynamic parameters.
Elderly
No specific dose adjustment recommended, but careful monitoring of cardiac function is advised.
Renal_impairment
No specific dose adjustment required. Monitor hemodynamic response.
How to Take
Dobutamine must be diluted before administration and given as a continuous intravenous infusion using an infusion pump. Dosage is individualized and adjusted according to patient response.
Mechanism of Action
Dobutamine directly stimulates beta-1 adrenergic receptors in the heart, increasing contractility and stroke volume. It has relatively minor effects on heart rate and systemic vascular resistance at therapeutic doses. It also has mild beta-2 and alpha-1 adrenergic activity.
Pharmacokinetics
Onset
1 to 10 minutes
Excretion
Excreted primarily in urine as metabolites and their conjugates.
Half life
2 minutes
Absorption
Administered intravenously, onset of action is rapid.
Metabolism
Metabolized rapidly in the liver and other tissues by catechol-O-methyltransferase (COMT) to inactive metabolites.
Side Effects
Contraindications
- Idiopathic hypertrophic subaortic stenosis
- Hypersensitivity to dobutamine or any component of the formulation
- Severe hypovolemia (unless corrected)
Drug Interactions
Beta-blockers
May blunt the inotropic response of dobutamine, requiring higher doses.
Nitroprusside
Concurrent use may produce a greater increase in cardiac output and decrease in pulmonary wedge pressure.
MAO Inhibitors (MAOIs)
May enhance the pressor effects, leading to hypertensive crisis.
Tricyclic Antidepressants
May potentiate the cardiovascular effects of dobutamine.
Storage
Store at controlled room temperature (20-25°C), protect from light. Do not freeze. Diluted solutions are stable for a limited time.
Overdose
Symptoms of overdose include excessive beta-stimulation (e.g., tachycardia, hypertension, myocardial ischemia). Management involves reducing the dose or discontinuing the infusion, instituting supportive care, and monitoring vital signs.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. It is not known whether dobutamine is excreted in human milk, exercise caution if administered to a nursing mother.

- Dobutamine injection is administered by trained healthcare professionals in a hospital or critical care setting.
- Patients will be closely monitored for vital signs and cardiac function during infusion.
- Report any discomfort, chest pain, or changes in breathing immediately.
Missed Dose Advice
Not applicable as Dobutamine is administered as a continuous infusion in a supervised clinical setting. Dosing is continuously monitored and adjusted by healthcare professionals.
Driving Precautions
Not applicable as patients receiving Dobutamine are typically in a critical care setting and not able to drive.
