Description
Overview of the medicine
Dorakid Syrup is used to treat nausea and vomiting, especially in children. It also helps in conditions where gastric motility is impaired.
Uses & Indications
Dosage
Adults
Adults and adolescents (≥12 years and ≥35 kg): 10 ml (10 mg) up to 3 times daily, maximum 30 ml (30 mg) per day.
Elderly
Similar to adult dosage, but caution with renal impairment.
Renal_impairment
Dosage reduction may be necessary; consult physician. In severe renal impairment (CrCl <30 mL/min), frequency should be reduced to once or twice daily depending on severity.
How to Take
Take orally, ideally 15-30 minutes before meals. Do not exceed the prescribed dose. Shake well before use.
Mechanism of Action
Domperidone, an active ingredient, works by blocking dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) and in the stomach. This increases gastrointestinal motility and produces an antiemetic effect.
Pharmacokinetics
Onset
Within 30-60 minutes for antiemetic effects.
Excretion
Approximately 66% excreted in feces and 33% in urine as inactive metabolites.
Half life
Approximately 7-9 hours, may be prolonged in severe renal impairment.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 30-60 minutes.
Metabolism
Extensively metabolized in the liver by CYP3A4 hydroxylation and N-dealkylation.
Side Effects
Contraindications
- Hypersensitivity to domperidone or any component of the syrup
- Known prolongation of cardiac QT intervals or significant cardiac disease (e.g., congestive heart failure)
- Moderate to severe hepatic impairment
- Prolactin-releasing pituitary tumour (prolactinoma)
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
- Gastrointestinal haemorrhage, mechanical obstruction, or perforation
Drug Interactions
Antacids or H2-receptor antagonists
May decrease the oral bioavailability of domperidone. Should be taken after domperidone.
Drugs that prolong QT interval (e.g., Amiodarone, Haloperidol, Quinidine)
Increased risk of cardiac arrhythmias. Concomitant use is contraindicated.
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Erythromycin, Clarithromycin)
Increased plasma levels of domperidone, significantly increasing the risk of QT prolongation. Concomitant use is contraindicated.
Storage
Store below 30°C, away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, disorientation, and extrapyramidal reactions, particularly in children. Management involves symptomatic and supportive treatment, including gastric lavage and activated charcoal if appropriate. ECG monitoring is recommended.
Pregnancy & Lactation
