Description
Overview of the medicine
Doximet 100 mg Capsule contains Doxycycline, which is a broad-spectrum tetracycline antibiotic used to treat various bacterial infections including respiratory tract infections, urinary tract infections, skin infections, sexually transmitted infections, rickettsial infections, and for malaria prophylaxis. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 200 mg on the first day (administered as 100 mg every 12 hours) followed by a maintenance dose of 100 mg once daily. For severe infections, 100 mg every 12 hours may be continued.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution is advised in those with significant hepatic impairment.
Renal_impairment
No dosage adjustment is needed in patients with impaired renal function, which is a unique advantage among tetracyclines.
How to Take
Take the capsule orally with a full glass of water (approx. 240 mL) to prevent esophageal irritation and ulceration. It can be taken with food or milk if gastric irritation occurs, but avoid dairy products, antacids, or iron preparations within 1-2 hours of taking Doximet, as they can interfere with absorption. Remain in an upright position for at least 30 minutes after taking the medicine.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit of susceptible bacteria, thereby preventing the attachment of aminoacyl-tRNA to the ribosomal acceptor site. This action prevents the elongation of the peptide chain, leading to bacteriostatic effects.

Onset
Peak plasma concentrations are usually reached within 1-3 hours after oral administration.
Excretion
Mainly excreted in urine and feces as inactive compounds, with approximately 40% eliminated via the kidneys within 72 hours and a significant portion via the gastrointestinal tract.
Half life
Plasma half-life is 16-22 hours, allowing for once or twice daily dosing.
Absorption
Well absorbed orally (approximately 90-100%) in the fasting state. Absorption is not significantly affected by food, although dairy products and antacids may reduce it.
Metabolism
Minimal hepatic metabolism; primarily undergoes enterohepatic recirculation.

Contraindications
- Hypersensitivity to Doxycycline or any other tetracycline antibiotics.
- Pregnancy (especially during the latter half) due to potential for permanent tooth discoloration and bone growth retardation in the fetus.
- Lactation, as it passes into breast milk.
- Children under 8 years of age (risk of permanent tooth discoloration).
Drug Interactions
Warfarin
Doxycycline may potentiate the anticoagulant effect of warfarin, requiring monitoring of INR and possible dose adjustment.
Penicillins
Tetracyclines can interfere with the bactericidal action of penicillins. Avoid concomitant use.
Methotrexate
May increase the serum concentration of methotrexate, potentially increasing toxicity.
Oral contraceptives
May reduce the effectiveness of oral contraceptives. Advise patients to use an alternative or additional method of contraception.
Retinoids (e.g., isotretinoin)
Risk of increased intracranial pressure (pseudotumor cerebri). Avoid concomitant use.
Antacids containing aluminum, calcium, magnesium, and iron preparations
Can decrease the absorption of Doxycycline. Take Doxycycline 1-2 hours before or 2-3 hours after these products.
Storage
Store in a cool, dry place (below 30°C), away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, and diarrhea. In case of overdose, discontinue the drug, institute gastric lavage, and provide supportive therapy. Hemodialysis is not effective in removing Doxycycline.
Pregnancy & Lactation
Doxycycline is contraindicated in the second and third trimesters of pregnancy due to the risk of permanent tooth discoloration and inhibition of bone growth in the fetus. It passes into breast milk and is generally not recommended during lactation. Alternative antibiotics should be considered.
