Overview of the medicine
Doxycycline is a broad-spectrum tetracycline antibiotic used to treat a wide variety of bacterial infections, including respiratory tract infections, urinary tract infections, skin infections, sexually transmitted infections, and certain parasitic infections. It works by inhibiting bacterial protein synthesis.
Uses & Indications
Dosage
Adults
Typical adult dose for most infections: 100 mg orally every 12 hours on the first day, followed by 100 mg once daily. For severe infections, 100 mg every 12 hours. For acne: 50-100 mg once daily.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in those with significant hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment, as doxycycline is primarily excreted non-renally.
How to Take
Take Doxseem capsules with a full glass of water (approx. 240 mL) to reduce the risk of esophageal irritation and ulceration. It can be taken with food or milk to minimize gastrointestinal upset, but avoid taking it with dairy products, antacids, or iron supplements within 2 hours of dose due to reduced absorption. Remain upright for at least 30-60 minutes after taking the capsule.
Mechanism of Action
Doxycycline inhibits bacterial protein synthesis by reversibly binding to the 30S ribosomal subunit of susceptible bacteria, thereby preventing the attachment of aminoacyl-tRNA to the ribosomal A site. This leads to the inhibition of peptide chain elongation and bacterial growth.

Onset
Antibacterial effects typically begin within 1-2 hours after administration.
Excretion
Primarily excreted unchanged in the feces via biliary and intestinal secretion. A smaller portion is excreted in the urine.
Half life
Plasma elimination half-life ranges from 16 to 22 hours, which allows for once or twice daily dosing.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration, with bioavailability typically between 90-100%. Peak plasma concentrations are reached in 2-4 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites. About 20% of the dose is metabolized.

Warfarin

Overdose
In case of overdose, gastrointestinal symptoms (nausea, vomiting, diarrhea) are most common. Management includes gastric lavage if ingestion is recent, followed by symptomatic and supportive treatment. Dialysis does not effectively remove doxycycline from the blood.
Pregnancy & Lactation
Doxycycline is contraindicated during pregnancy due to the risk of permanent tooth discoloration and skeletal effects in the fetus. It is also contraindicated during lactation as it is excreted in breast milk and can cause similar adverse effects in nursing infants.
