Overview of the medicine
Duofix-375 mg tablet is a combination antibiotic containing Cefixime (a 3rd generation cephalosporin) and Clavulanic Acid (a beta-lactamase inhibitor). It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
The usual adult dose is one Duofix-375 mg tablet (Cefixime 250 mg + Clavulanic Acid 125 mg) orally every 12 hours for 7 to 14 days, depending on the severity and type of infection.
Elderly
No specific dose adjustment is generally required for elderly patients with normal renal function. Dose should be adjusted in case of renal impairment.
Renal_impairment
Dose adjustment is necessary for patients with severe renal impairment (creatinine clearance < 30 mL/min). Consult a physician for specific recommendations.
How to Take
Take the tablet orally with a glass of water, with or without food. It is recommended to take it at regular intervals.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis, leading to bacterial cell lysis and death. Clavulanic Acid inactivates a wide range of beta-lactamase enzymes produced by bacteria, which would otherwise degrade Cefixime, thereby extending Cefixime's antibacterial spectrum.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved in approximately 2-6 hours for Cefixime and 1-2 hours for Clavulanic Acid.
Excretion
Cefixime is primarily eliminated by renal excretion. Clavulanic Acid is also largely excreted renally.
Half life
Cefixime has a half-life of 3-4 hours. Clavulanic Acid has a half-life of approximately 1 hour.
Absorption
Both Cefixime and Clavulanic Acid are well absorbed from the gastrointestinal tract after oral administration. Cefixime's bioavailability is approximately 40-50%. Clavulanic acid is also well absorbed.
Metabolism
Cefixime undergoes limited hepatic metabolism. Clavulanic Acid is extensively metabolized.
Side Effects
Contraindications
- Hypersensitivity to Cefixime, Clavulanic Acid, other cephalosporins, penicillins, or any component of the formulation.
- History of severe allergic reactions to beta-lactam antibiotics.
Drug Interactions
Probenecid
Increases plasma concentrations of Cefixime by decreasing its renal tubular secretion.
Carbamazepine
Concomitant administration may increase plasma levels of carbamazepine.
Oral Anticoagulants (e.g., Warfarin)
May increase INR and risk of bleeding. Close monitoring is advised.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. No specific antidote exists. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Small amounts of Cefixime are excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult your doctor.
