Overview of the medicine
Eplerenone is a selective aldosterone receptor antagonist used to treat heart failure after a heart attack and high blood pressure.
Uses & Indications
Dosage
Adults
For hypertension, initial 50 mg once daily, adjusted based on response up to 50 mg twice daily. For heart failure post-MI, initial 25 mg once daily, titrated to 50 mg once daily within 4 weeks.
Elderly
No specific dosage adjustment usually needed, but monitor renal function.
Renal_impairment
Contraindicated in severe renal impairment. Dose adjustment needed for moderate impairment (e.g., initial 25 mg every other day).

Oral, with or without food.
Mechanism of Action
Eplerenone selectively binds to the mineralocorticoid receptor, blocking the binding of aldosterone. This prevents aldosterone-mediated sodium reabsorption and potassium excretion, leading to diuresis and reduction in blood pressure.

Contraindications

Drug Interactions
NSAIDs

Overdose
